区域协同医护一体化无缝链接模式提高老年急性缺血性脑卒中救护效果

摘　　要：目的探讨区域协同医护一体化无缝链接模式在老年急性缺血性脑卒中救治中的可行性及实施效果。方法对上海宝山区某7家二级网络医院老年急性缺血性脑卒中162例患者,采用数字随机分组法分为对照组85例和试验组77例,对照组采用区域协同救治方案,试验组在对照组基础上实施医护一体无缝链接模式护理干预,比较2组患者救治时间点、护士工作效率及临床结局指标。结果试验组与对照组患者首次医疗接触至脑卒中干预时间(19.16±2.69)min vs(32.81±3.81)min、到达医院至血管再通时间(156.48±10.72) min vs (208.55±6.11)min、转诊时间(74.21±10.86)min vs (98.61±8.32)min及获取知情同意时间、NIHSS评分、症状性脑出血发生率及病死率比较,差异具有统计学意义(P<0.05)。结论区域协同医护一体化无缝链接模式干预能够缩短老年急性缺血性脑卒中患者区域化协同救治时间,预后等得到改善,提高老年急性缺血性脑卒中的救治成功率。Objective To discuss the feasibility and the effect of seamless link model of regional collaborative medical integration in the rescue of the elderly with acute ischemic stroke.Methods 162 elderly with acute ischemic stroke were randomly divided into 2 groups:control group(n=85)and experiment group(n=77);regional cooperative rescue was performed on the cases in both groups while nursing intervention of seamless link model of medical integration was added to cases in experiment group;a comparative study was made in rescue time point,the efficiency of nursing and clinical outcomes between the 2 groups.Results The time from first medical contact to stroke intervention time,the time from reaching hospital to vascular recanalization and the time of referral in experiment group were(19.16±2.69)min,(156.48±10.72)min and(74.21±10.86)min respectively while those in control group were(32.81±3.81)min,(208.55±6.11)min and(98.61±8.32)respectively;the comparison between the 2 groups in the time of obtaining informed consent,NIHSS scores,the incidence of symptomatic cerebral hemorrhage and the mortality rate was of statistical difference(P<0.05).Conclusions Intervention with seamless link model of regional collaborative medical integration can shorten the time in rescuing the elderly with acute ischemic stroke,improve the prognosis and raise the successful rescue rate.
