摘　　要：目的系统性评价血红蛋白浓度对慢性肾脏病患者健康状况的影响。方法计算机检索Pubmed、Cochrane、Embase、Web of Science、中国知网、万方数据库、维普网,收集所有关于治疗肾性贫血过程中血红蛋白浓度对患者健康状况(用健康状况量表SF-36评价)影响的随机对照试验,应用Cochrane协作网提供的Revman软件对结果进行Meta分析。结果最终纳入8项研究,包括2638例患者。Meta分析结果显示:观察合并效应量[MD=-6.89,95%CI(-9.13,-3.02),P<0.01]表明血红蛋白100～110 g/L组健康状况优于90～100 g/L,差异有统计学意义;观察合并效应量[MD=-1.58,95%CI(-3.24,0.08),P=0.06]表明血红蛋白100～110 g/L和110～120 g/L的健康状况差异无统计学意义。结论血红蛋白浓度达到100 g/L以上时,患者即具有较好的健康相关生命质量。Objective To systematically evaluate the effect of hemoglobin concentration on the health status of patients with chronic kidney disease. Methods Pubmed, Cochrane, Embase, Web of Science, China Knowledge Network, Wanfang Database, and Weipu were searched by computer. A randomized controlled trial of the effects of hemoglobin concentrations on the patient's health status during the treatment of renal anemia was collected(evaluated with the Health Status Scale SF-36). A Meta-analysis of the results using the Revman software provided by the Cochrane Collaboration was performed. Results Eight studies were eventually included, including 2638 patients. Meta-analysis results showed that the effect size [MD=-6.89, 95% CI(-9.13,-3.02), P<0.01] showed that the health status in hemoglobin 100-110 g/L group was better than that of 90-100 g/L group, and the difference was statistically significant. Effect size [MD=-1.58, 95% CI(-3.24, 0.08), P=0.06] showed that the difference in health between hemoglobin 100-110 g/L and 110-120 g/L was not statistically significant. Conclusion When the hemoglobin concentration reaches 100 g/L or more, the patient has a good health-related quality of life.
