摘　　要：目的探讨改良胆肠袢式吻合术对先天性胆总管囊状扩张症的应用及对患者QOL评分的影响。方法选取2005年2月～2017年9月我院收治的43例先天性胆总管囊状扩张症患者,随机分为两组,对照组应用胆肠Rouxen-Y吻合术治疗,研究组应用改良胆肠袢式吻合术治疗。比较两组患者手术及术后恢复情况、治疗前后血生化指标和治疗前后QOL评分。结果研究组手术平均时间、肠鸣音恢复时间和肛门第一次排气时间短于对照组,同时术中出血量低于对照组(P<0.05),治疗后,研究组ALT和CRP指标低于对照组,同时GGT指标高于对照组(P<0.05),治疗后,研究组患者的QOL评分显著低于对照组(P<0.05)。结论先天性胆总管囊状扩张症的治疗过程中,患者采用改良胆肠袢式吻合术的治疗方法效果显著,临床上应进一步推广和应用。Objective To explore the application of modified cholangio-intestinal loop anastomosis for congenital cystic dilatation of the common bile duct and its influence on QOL score of patients. Methods Forty-three patients with congenital choledochal cystic dilatation admitted to our hospital from February 2005 to September 2017 were randomly divided into two groups. The control group was treated with cholestasis Roux-en-Y anastomosis. The study group was treated with modified cholangio-intestinal loop anastomosis. The operation and postoperative recovery, blood biochemical parameters before and after treatment, and QOL scores before and after treatment were compared between the two groups. Results The average operation time, the recovery time of the bowel sounds and the first exhaust time of the anus in the study group were shorter than those of the control group, and the intraoperative blood loss was lower than that of the control group(P<0.05). After treatment, the study group had lower ALT and CRP indexes and higher GGT index than the control group (P<0.05). After treatment, the QOL score of the study group was significantly lower than that of the control group(P<0.05). Conclusion In the treatment of congenital choledochal cystic dilatation, the effect of modified cholangio-intestinal loop anastomosis is more significant, so it should be further promoted and applied clinically.
