超声筛查对胎儿畸形的诊断及危险因素分析

摘　　要：目的研究产前超声筛查对胎儿畸形的临床诊断应用及危险因素分析。方法对该院近五年10 512例孕妇进行产前检查,畸形检查项目主要包括神经、循环、消化、泌尿、骨骼等系统畸形及颜面畸形,并与最终的胎儿临床引产和经娩出进行证实,分析超声对产前检查的准确性及对相关危险因素进一步多因素Logistic回归分析。结果共筛查出298例胎儿畸形,分别累及以下系统:心脏72例、泌尿系统61例、骨骼55例、消化系统50例、神经系统45例、颜面部15例,超声检查的准确率为92.3%。TORCH感染,年龄及感冒均为胎儿畸形发生的独立危险因素。结论产前B超筛查对胎儿畸形具有较高的检出率,是一种方便、实惠、实用性很强的产前胎儿畸形筛查方法,可以提升优生质量。Objective To study the clinical diagnosis and risk factors analysis of fetal malformation by prenatal ultrasound screening.Methods 10 512 pregnant women in our hospital for antenatal examination in the past five years were selected.The malformation examination items mainly include nervous,circulatory,digestive,urinary,skeletal and other system malformations and facial deformities,and confirmed with the final fetal clinical induction and delivery.Analyze the accuracy of prenatal examination with ultrasound and further multivariate logistic regression analysis of relevant risk factors.Results A total of 298 fetal malformations were screened and the following systems were involved:72 cases of heart,61 cases of urinary system,55 cases of bone,50 cases of digestive system,45 cases of nervous system,15 cases of facial surface,the accuracy of ultrasound examination 92.3%.TORCH infection,age and cold are independent risk factors for fetal malformation.Conclusion Prenatal B-ultrasound screening has a high detection rate for fetal malformation.It is a convenient,affordable and practical method for prenatal fetal malformation screening,which can improve the quality of eugenics.
