草木樨流浸膏片联合督灸对HLA-B27阳性强直性脊柱炎患者Th1、Th2和Th17的影响

摘　　要：目的:观察草木樨流浸膏片联合督灸对HLA-B27阳性强直性脊柱炎(AS)患者Th1、Th2、Th17细胞和血清炎症因子的影响,探讨草木樨流浸膏片对AS的药理作用。方法:将40例HLA-B27阳性AS患者随机分为两组,试验组给予草木樨流浸膏片1mg/kg,tid联合督灸治疗,对照组给予双氯芬酸钠片1mg/kg,tid,分别于治疗前及治疗4周后检测Th1、Th2和Th17细胞比例,用ELISA法检测血清细胞因子IL-2、IFN-γ、IL-4、IL-10。结果:①治疗前,Th1、Th2和Th17细胞比例差异无显著性(P>0.05);炎症因子水平无显著差异(P>0.05);②治疗4周后,所有受试者病情明显改善,其中与对照组相比,试验组Th1和Th17细胞比例明显降低(P<0.05);细胞因子IL-2和IFN-γ水平明显降低(P<0.05);IFN-γ/IL-4比值明显降低(P<0.05)。结论:草木樨流浸膏片联合督灸能调节AS患者Th1、Th2和Th17细胞比例,降低细胞因子IL-2和IFN-γ水平,并降低IFN-γ/IL-4的比值。Objective: To observe the potential effect of Caomuxiliuqingaopian with DuJiu on Th1, Th2 and Th17 cell and serum inflammatory cytokines (IL-2, IFN-γ, IL-4, IL-10)in patients of ankylosing spondylitis (AS) with positive HLA-B27. Methods: The 40 AS patients were randomly divided into 2 groups: treatment group and control group, the treatment group received Caomuxiliuqingaopian 1mg/kg,three times a day and DuJiu;the control group received diclofenac1mg/kg,three times a day and DuJiu. After 4 weeks of treatment, the proportions of Th1, Th2 and Th17 cell in peripheral blood were assayed;serum inflammatory cytokines of IL-2, IFN-γ, IL-4 and IL-10 were determined by radioimmunoassay method. Results: Before treatment, there was no significant difference in the proportion of Th1, Th 2 and Th17 cells (P>0.05);there was no significant difference in the level of inflammatory factors (P>0.05). After 4 weeks of treatment, the condition of all subjects was significantly improved, in which the proportion of Th1 and Th17 cells in the treatment group were significantly lower than that in the control group (P<0.05);The level of cytokines IL-2 and IFN-γ were obviously decreased (P<0.05)than those in the controls, and IFN-γ/IL-4 ratio (P<0.05)were significantly decreased. Conclusion: Caomuxiliuqingaopian can reduce serum inflammatory cytokines of TNF-α and IL-6,Caomuxiliuqingaopian is an ideal medicine in treating ankylosing spondylitis.
