丹参酮Ⅱ_A磺酸钠注射液致过敏性休克1例及148例不良反应文献复习

摘　　要：目的探讨丹参酮ⅡA磺酸钠注射液所致药品不良反应的一般规律和临床特点。方法分析医院应用丹参酮ⅡA磺酸钠注射液导致严重过敏性休克1例患者的临床资料;检索并分析2000年至2017年国内医药期刊公开报道的丹参酮ⅡA磺酸钠注射液所致不良反应。结果共检索到26篇文献、148例不良反应报道;不良反应在高龄组发生率较高;不良反应累及多个系统,临床表现复杂多样,主要表现为皮肤及其附件损害,严重者可出现过敏性休克;不良反应多发生在给药后30 min以内,绝大多数经治疗后转归良好。结论丹参酮ⅡA磺酸钠注射液会致过敏性休克等严重不良反应,应提高临床合理用药水平,加强用药监护,避免或减少不良反应的发生,保障用药安全。Objective To investigate the general regularity and clinical characteristics of adverse drug reactions(ADRs)induced by Sodium TanshinoneⅡA Sulfonate Injection.Methods The clinical data of one case of severe allergic shock induced by Sodium TanshinoneⅡA Sulfonate Injection in our hospital were analyzed.The ADRs induced by Sodium TanshinoneⅡA Sulfonate Injection reported in domestic medical journals from 2000 to 2017 were retrieved and analyzed.Results Totally 26 articles and 148 reports of ADRs were retrieved.The higher occurrence rate of ADRs was observed in the elderly group.Multiple organs and systems were involved in ADRs and the clinical manifestations were various and complex,the main manifestations were skin and its accessories damage,and allergic shock was observed in severe ADRs.The onset time of most ADRs was within 30 min after medication and most ADRs had good prognosis after treatment.Conclusion Sodium TanshinoneⅡA Sulfonate Injection could cause severe ADRs,such as allergic shock.It is necessary to improve the level of rational drug use and strengthen drug monitoring in order to avoid or reduce the occurrence of ADRs and ensure the patient′s medication safety.
