摘 要:目的分析反复输血对血液病患者血小板输注效果的影响。方法选取我院2016年8月至2017年10月行反复输血治疗的血液病患者69例,行血小板抗体检测(固相凝集法),筛查出血小板抗体阴性和阳性的患者,比较不同输血次数患者的输注无效(PTR)发生率和血小板抗体阳性率,比较血小板抗体阴性和抗体阳性患者的一般资料、血小板输注效果及非溶血性输血反应(NHTR)发生情况。结果输血次数为6次以上患者的PTR发生率和血小板抗体阳性率均高于输血次数为3~6次的患者(P<0.05);血小板抗体阳性患者所占比例为42.03%;血小板抗体阳性患者的平均年龄大于血小板抗体阴性患者,平均输血次数多于血小板抗体阴性患者,白血病患者占比高于血小板抗体阴性患者(P<0.05);血小板抗体阳性患者PRT发生率和FNHTR发生率均高于血小板抗体阴性患者(P<0.05)。结论血液病患者反复输血易产生血小板抗体,而血小板抗体的产生会增加血小板输注时PTR发生率及FNHTR率发生率。Objective To analyze the effect of repeated blood transfusions on platelet transfusion efficiency in patients with blood diseases. Methods A total of 69 patients with blood diseases who underwent repeated blood transfusions in our hospital from August 2016 to October 2017 were selected, and platelet antibody tests(solid phase agglutination method)were performed to screening patients with negative platelet antibody or positive platelet antibody. The incidences of platelet transfusion refractory(PTR) and the positive rates of platelet antibody in patients with different transfusion times were compared. The general information, platelet transfusion efficiency and the non-hemolytic transfusion reaction(NHTR)between patients with negative platelet antibody and positive platelet antibody were compared. Results The incidence of PTR and the positive rate of platelet antibody in patients who underwent the blood transfusions more than 6 times were higher than those in patients who underwent the blood transfusions for 3-6 times(P<0.05). The percentage of patients with positive platelet antibody was 42.03%. The average age of patients with positive platelet antibody was older than that of the patients with negative platelet antibody, the average number of blood transfusion was more than that of the patients with negative platelet antibody, and the proportion of leukemia patients was higher than that of the patients with negative platelet antibody(P<0.05). The incidences of PRT and FNHTR in patients with positive platelet antibody was higher than those in patients with negative platelet antibody(P <0.05). Conclusion Platelet antibodies are easily produced in blood diseases patients with repeated blood transfusions, which will increase the incidence of PTR during platelet transfusions.
