同型半胱氨酸与类风湿关节炎相关抗体及炎症指标的关系

摘　　要：目的研究类风湿关节炎(RA)合并心血管病变(CVD)患者同型半胱氨酸水平与相关抗体及炎症指标的关系。方法选取2015年1月～2017年12月在我院住院的RA患者160例,根据患者是否同时合并心血管病变分为RA合并CVD组(n=80)及RA未合并CVD组(n=80)。同时选取健康对照者80例作为正常对照组。分别检测3组患者的血清同型半胱氨酸、类风湿因子(RF)、抗环瓜氨酸肽抗体(ACCP)、血沉(ESR)、C-反应蛋白(CRP)、胱抑素C、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)、免疫球蛋白A(IgA)等相关指标,同时记录所有试验对象的年龄、病程、体重、是否合并高血压、胆固醇及甘油三酯水平,分析RA合并CVD患者同型半胱氨酸与各项指标之间的相关性及三组同型半胱氨酸水平的差异。结果 RA合并CVD患者同型半胱氨酸水平与CRP、ESR、胱抑素C呈正相关(P<0.05)。RA合并CVD组血清同型半胱氨酸水平高于RA未合并CVD组(P<0.05)。结论 RA合并CVD患者同型半胱氨酸水平与RA疾病活动度及CVD发生密切相关。Objective To study the effect of serum homocysteine on rheumatoid arthritis with cardiovascular disease(CVD) and the relation with related antibody and inflammatory markers. Methods A total of 160 patients with RA were enrolled in our hospital from Jan 2015 to Dec 2017. All patients were divided into two groups(RA with CVD group and RA without CVD group) according to whether or not associated with CVD, 80 cases in each group. At the same period, 80 normal subjects were selected as the control group. The level of homocysteine, rheumatoid factor(RF), anti cyclic citrullinated peptide antibody(ACCP), erythrocyte sedimentation rate(ESR), C-reactive protein(CRP), cystatin C, immunoglobulin G(IgG), Immunoglobulin M(IgM), and immunoglobulin A(IgA) in 3 groups were tested. The age, course of disease, weight, hypertension, cholesterol and triglycerides were recorded in all cases. The correlation between homocysteine and various indexes was analyzed, the differences of homocysteine in 3 groups were analyzed. Results The level of homocysteine in RA with CVD group was positively correlated with CRP, ESR, cystatin C(P<0.05). The level of homocysteine in RA with CVD group was higher than that in RA without CVD group(P<0.05). Conclusion The level of homocysteine is closely related with disease activity and CVD in patients with RA.
