摘  要:目的探讨剪切波弹性成像(SWE)和超微血流成像(SMI)对乳腺癌的诊断价值。方法收集2016年7月至2018年3月江苏省肿瘤医院及南京市江宁医院收治并于术前完成SWE及SMI的乳腺肿瘤患者78例(88个病灶)的临床资料,SWE获得杨氏模量(E)定量分析,SMI获得病灶血流Alder分级,并与病理对照,研究SWE及SWE联合SMI对乳腺癌的诊断价值。结果良性病灶E平均值为30.6 kPa(19.6~45.3 kPa),恶性病灶E平均值为55.4 kPa(44.6~121.8 kPa),两者差异有统计学意义(P<0.05)。乳腺癌Alder 0~Ⅲ级分别占40.91%(9/22)、22.73%(5/22)、62.50%(10/16)、78.57%(22/28)。SWE与SWE联合SMI的敏感性、特异性、准确性分别为86.95%、88.10%、87.50%和91.30%、92.86%、92.05%,两者相比,差异有统计学意义(P<0.05)。结论 SWE和SMI是有效诊断乳腺癌的手段,两者联合应用可提高乳腺癌诊断的敏感性、特异性、准确性。Objective To explore the diagnostic value of shear wave elastography (SWE) and super microvascular imaging (SMI) for breast cancer. Methods During July 2016 to March 2018,78 breast cancer patients with 88 lesions who finished both SWE and SMI before opreation were selected in this study from Jiangsu Cancer Hosital and Nanjing Jiangning Hospital.The Young's modulus (E) and Alder scale were calculated by SWE and SMI respectively.Compared to the pathological diagnosis,the diagnostic value of SWE and SWE+SMI were analyzed. Results The average E of benign and melignant lesions were 30.6 kPa(19.6-45.3 kPa)and 55.4 kPa(44.6-121.8 kPa).There was significant difference between the two lesions ( P < 0.05).The Alder scale from 0 to Ⅲ accounted for 40.91%(9/22),22.73%(5/22),62.50%(10/16) and 78.57%(22/28),respectively.The sencitivity,specitivity,accuracy of SWE and SWE combined with SMI was 86.95%,88.10%,87.50% and 91.30%,92.86%,92.05%.The combination was superior to SWE alone( P < 0.05). Conclusions SWE and SMI are both effective methods for diagnosing of breast cancer.And the combination of two methods was superior to SWE alone for breast cancer.
