心理健康及睡眠质量与ERSD继维持性血液透析临床相关性分析

摘　　要：目的 探讨维持性血液透析(Maintenance hemodialysis,MHD)对患者心理健康损害及睡眠质量影响因素的相关性。方法 通过汉密尔顿抑郁量表(HAMD)、心里健康自评量表(Scl-90)、匹茨堡睡眠质量指数量表(PSQI)问卷调查将其分为正常对照组、轻度抑郁睡眠较好组和中重度抑郁睡眠较差组,统计分析患者心理健康及睡眠质量的影响因素。结果 绝大部分患者有抑郁情绪且发生抑郁情绪为91.83%,MHD患者抑郁发生频率为(4.04±1.12)。通过对3组患者心理健康数据统计学分析发现,中重度抑郁睡眠较差组受抑郁、焦虑症状影响(23.73±1.71,22.18±2.12;P<0.001);轻度抑郁睡眠较好组受焦虑症状影响(14.56±1.03;P<0.05);中重度抑郁睡眠较好组受到偏执、精神病性影响(8.33±0.45,8.52±1.23;P<0.05);中重度抑郁睡眠较好组受到人际关系敏感影响(14.35±1.12;P<0.01)。绝大部分MHD患者具有睡眠质量问题(PSQI>7),其中受到睡眠时间、睡眠障碍及日间功能障碍的影响(P<0.05),入睡时间、睡眠效率及催眠药物可能影响睡眠质量,但该研究无统计学差异(P>0.05)。结论 维持性血液透析患者临床心理健康及睡眠质量受到严重威胁,适当的心理疏导和提高睡眠质量等其他干预方法对终末期肾脏病患者身心健康有重要意义。Objective To explore the correlation on MHD patients' mental health and analyze the relevance of influencing factors on sleep quality. Methods Ninety-eight cases of MHD patients were by Hamilton Depression Scale (HAMD), Mental health self rating scale (Scl-90), Pittsburgh sleep quality index (PSQI) questionnaire.They will be divided into mild depression-sleep better group,severe depression-poor sleep group and normal control group (without depression, sleep better group), statistical score was determined the factors affecting the quality of sleep in patients with mental health. Results The incidence of depression was as high as 91.83%, and the frequency of depression in MHD patients was 4.04±1.12. MHD patients with severe depression sleep poor mental health group was effected by depression, anxiety (23.73±1.71,22.18±2.12;P<0.001);mild depression sleep better group was effected by anxiety symptoms (14.56±1.03;P<0.05);severe depression better sleep group was also effected by paranoia and psychosis (8.33±0.45,8.52±1.23;P<0.05);and severe depression better sleepgroup was also effected by interpersonal sensitivity (14.35±1.12;P<0.01). MHD patients have sleep quality problems (PSQI>7), which were affected by sleep time, sleep disorders and daytime dysfunction (P<0.05). Sleep time, sleep efficiency and hypnotic drugs may affect the quality of sleep, but there was no statistical difference in this study (P>0.05).Conclusion The mental health and sleep quality of maintenance hemodialysis patients are seriously threatened.Proper psychological counseling,improving sleep quality and other intervention methods are important for patients' physical and mental health.
