摘 要:目的探讨预防四级手术医疗纠纷新措施在实践中的作用。方法将400例进行四级手术的患者随机分为2组。旧措施组采用手术医师常规术前谈话签字的处理方式,新措施组采用"手术医师常规术前谈话签字""医疗安全信息系统排摸纠纷隐患病人""重大疑难手术术前行政谈话""手术意外险""执业医师险"多种方式叠加的处理方式。比较2组处理后的医疗纠纷发生率、手术医师对预防纠纷前置处理流程满意度率、患方对预防纠纷前置处理流程满意率。结果新措施组纠纷发生率低于传统组(P <0. 05),新措施组的手术医师、患方对预防纠纷前置处理流程满意率均高于旧措施组(P <0. 05)。结论预防四级手术医疗纠纷新措施对于预防医疗纠纷、手术医师及患方对流程满意率的改善方面优于旧措施处理,值得向整个医疗卫生行业推广。Objective: To explore the role of new measures to prevent grade-four surgical disputes in practice. Methods:400 patients undergoing grade-four surgery were randomly divided into two groups.The former group adopted the treatment of conventional preoperative talk and signature of surgeons,while the new group adopted the treatment of multiple ways of superposition of “conventional preoperative talk and signature of surgeons”,“medical security information system to remove hidden trouble patients”,“major difficult preoperative administrative talk”,“surgical accident insurance” and “medical practitioner insurance”.The incidence rate of medical disputes,the satisfaction rate of surgeons with the pre-treatment process for preventing disputes,and the satisfaction rate of patients with the pre-treatment process for preventing disputes were compared. Results:The incidence of disputes in the new measure group was lower than that in the traditional measure group (P<0.05),and the satisfaction rate of surgeons and patients in the new measure group with the pre-treatment process was higher than that in the old measure group (P<0.05). Conclusion:The new measures for the prevention of grade-four surgical medical disputes and the improvement of the satisfaction rate with the procedure by surgeons and patients are better than the old ones,which is worth promoting to the whole medical and health industry.
