降脂通脉汤内服联合大黄敷脐治疗冠心病伴高血脂症疗效及对血清Hcy、IL-6、APN和血脂指标的影响

摘　　要：目的观察降脂通脉汤内服联合大黄敷脐治疗冠心病伴高血脂症疗效及对血清同型半胱氨酸(Hcy)、白细胞介素-6(IL-6)、脂联素(APN)水平和血脂指标的影响。方法将110例冠心病伴高血脂症患者随机分为研究组和对照组各55例,2组均给予常规西药治疗,在此基础上对照组给予大黄敷脐治疗,研究组在对照组治疗基础上联合降脂通脉汤治疗,均治疗4周。观察2组治疗前后中医证候积分、三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)和血清Hcy、IL-6、APN水平变化情况,统计2组临床疗效。结果治疗后2组中医证候积分和TG、TC、LDL-C、Hcy、IL-6水平均显著低于治疗前(P均<0.05),且研究组显著低于对照组(P均<0.05);2组HDL-C、APN水平均显著高于治疗前(P均<0.05),且研究组显著高于对照组(P均<0.05)。研究组总有效率显著高于对照组(P<0.05)。结论降脂通脉汤内服联合大黄敷脐可有效调节冠心病伴高血脂症患者血脂水平和血清Hcy、IL-6、APN水平,提高临床疗效。Objective It is to observe the curative effect of Jiangzhi Tongmai Decoction combined with umbilical compress therapy with Dahuang on coronary heart disease with hyperlipidemia and its effect on serum homocysteine (Hcy), interleukin-6 (IL-6), adiponectin (APN) and blood lipids. Methods 110 patients with coronary heart disease and hyperlipidemia were randomly divided into study group and control group, 55 cases in each group. Both groups were treated with conventional western medicine. On the basis of this, the control group was treated with umbilical compress therapy with Dahuang, and the study group was treated with Jiangzhi Tongmai Decoction. Both groups were treated for 4 weeks. The clinical efficacy was assessed after 4 weeks of treatment. The changes of TCM syndrome scores, the levels of triglyceride (TG), total cholesterol (TC), high density lipoprotein (HLD-C), and low density lipoprotein (LDL-C) and serum levels of Hcy, IL-6, and APN were observed before and after treatment in the two groups. Results The scores of TCM syndrome and the levels of TG, TC, LDL-C, Hcy and IL-6 after treatment in the two groups were significantly lower than those before treatment ( P <0.05), the scores and levels in the study group were significantly lower than those in the control group ( P < 0.05);the levels of HLD-C and APN in the two groups were significantly higher than those before treatment ( P <0.05), and that in the study group was significantly higher than the control group ( P <0.05). The total effective rate of the study group was significantly higher than that of the control group ( P <0.05). Conclusion Jiangzhi Tongmai Decoction combined with umbilical compress therapy with Dahuang can effectively regulate blood lipid levels and serum levels of Hcy, IL-6 and APN and improve clinical efficacy in patients with coronary heart disease and hyperlipidemia.
