摘 要:目的探讨血清胱抑素C(Cys C)和同型半胱氨酸(Hcy)水平变化在2型糖尿病肾病(DN)早期诊断中的意义。方法选取2017年1—12月入住本院的2型糖尿病患者(T2DM) 100例,根据24 h尿白蛋白排泄率(UAER)水平将患者分为单纯糖尿病组(DM组,n=52)、早期DN组(EDN组,n=48),另选取同期健康体检者为正常对照组(n=40)。检测并比较各组受检者的血清Cys C、Hcy、肌酐(Cr)、尿素氮(尿素)、UAER的水平。结果 DM组、EDN组血清Cys C、Hcy呈现增高趋势,与对照组间差异显著(P<0.05),且随着病情的加重血清Cys C、Hcy升高愈明显(P<0.05)。DM组、EDN组血清Cys C、Hcy阳性率明显高于对照组,差异有统计学意义(P<0.05),EDN组中血清Cys C、Hcy阳性率明显高于DM组,差异有统计学意义(P<0.05)。相关分析结果显示,血清Cys C、Hcy水平与UAER均呈正相关性(r=0.631、0.555,P<0.05)。Cr、尿素水平与UAERF无明显的相关性。结论血清Cys C、Hcy均能较敏感反映早期肾损伤,可作为糖尿病肾病患者早期肾损伤的诊断指标。Objective Investigating the significance of level changes of serum cystatin C(Cys C)and homocysteine(Hcy)in the early diagnosis of diabetic nephropathy(DN).Methods Selecting 100 patients with type 2 diabetes mellitus(T2 DM),and dividing them into simple diabetes mellitus group(DM group,n=52)and early DN group(EDN group,n=48)according to 24-hour urinary albumin excretion rate(UAER).Another 40 healthy people of the corresponding period were also selected.Checking and comparing the levels of serum Cys C,Hcy,creatinine(Cr),urea nitrogen(urea)and UAER in each group.Results The levels of Cys C and Hcy in DM and EDN groups were significantly higher than those in control group(P<0.05).With the aggravation of the disease,the serum Cys C and Hcy increased significantly(P<0.05).The positive rates of serum Cys C and Hcy in group DM and group EDN were significantly higher than those in control group.The differences were statistically significant(P<0.05).The positive rates of serum Cys C and Hcy in group EDN were significantly higher than those in DM group.The differences were statistically significant(P<0.05).Relevance analysis showed that serum Cys C and Hcy levels were positively correlated with UAER(r=0.631&0.555,P<0.05).There was no significant correlation between Cr,urea level and UAERF.Conclusions Both serum Cys C and Hcy could reflect early renal injury sensitively,and could be used as a diagnostic index of early renal injury in patients with diabetic nephropathy.
