括约肌间瘘管结扎术治疗复杂性肛瘘的临床疗效分析

摘　　要：为探讨括约肌间瘘管结扎术治疗复杂性肛瘘的临床疗效,回顾2016年8月至2017年8月我院收治的88例复杂性肛瘘患者资料,其中45例行括约肌间瘘管结扎术治疗(研究组),43例行切开挂线术治疗(对照组)。对比2组患者总体疗效、手术时间、术中出血量、术后疼痛程度、住院时间、创口愈合时间,以及术前与术后1个月、6个月时肛门功能。结果显示,研究组手术时间长于对照组,P <0.05;2组治愈率比较差异无统计学意义,P>0.05;但研究组术中出血量、术后3d内疼痛评分、住院时间、创面愈合时间,以及术后1个月和6个月肛门功能评分均明显少于对照组,P <0.05。结果表明,括约肌间瘘管结扎术治疗复杂性肛瘘疗效确切,虽手术时间略长于切开挂线术,但患者术后恢复更快,对肛门功能影响更小,更适于临床应用。To investigate the clinical effect of ligation of intersphincteric fistula tract(LIFT)for treating complex fistula in ano,author retrospectively analysed the data of 88cases who had been treated in author 's hospital(2016-08-2017-08),included 45cases(as research group,received LIFT)and 43cases(as control group,received incision &thread-drawing procedure);then compared following indexes such as whole effect,operative time,intraoperative bleeding volume,postoperative pain degree,hospitalization time, wound healing time,as well as anal function before surgery,and at 1,6months after srugery between the two groups.As results,in operative time was longer than control group(P <0.05);in cure rate there was no statistical difference between the two groups(P >0.05),and in intraoperative bleeding volume,score on pain within 3days after surgery,hospitalization time,wound-surface healing time,as well as score on anal function at 1and 6months after surgery,observation group was respectively and significantly less than control group(all,P <0.05).Results show that compared with incision &thread-drawing procedure LIFT for treating complex anal fistula can achieve better effect,postoperative recovery faster,less influence on anal function,though slight long operative time,thus it is more suitable to apply clinically.
