摘　　要：目的探讨采用带尺骨瓣的游离尺动脉腕上皮支复合皮瓣修复手指复合组织缺损的安全性和有效性。方法回顾性分析2013年6月-2018年4月采用带尺骨瓣的游离尺动脉腕上皮支复合皮瓣修复的10例手指复合组织缺损患者资料。结果 10例皮瓣全部成活,未出现血管危象、皮缘坏死等情况。术后4～6周拔出内固定克氏针,其中9例获得随访,随访时间6～24个月。手指功能恢复好,指骨与尺骨瓣愈合良好;皮瓣颜色接近正常手指皮肤;感觉恢复良好;前臂旋转、负重及腕关节屈伸功能均无影响。结论带尺骨瓣的游离尺动脉腕上皮支复合皮瓣修复手指复合组织缺损,具有皮支恒定、不损伤主干血管、骨组织愈合快、切取方便灵活、手指功能外观恢复好等优点,是修复手指复合组织缺损方便且有效的方法。Objective To evaluate the safety and effectiveness of free flap pedicled with wrist cutaneous branch of ulnar artery to repair complex tissue defects of fingers. Methods A retrospective study was carried out from June 2013 to April in 2018 to repair 10 cases of finger complex tissue defect with free flap pedicled with wrist cutaneous branch of ulnar artery combined with ulnar flap. Results All 10 skin flaps survived, without vascular crisis and necrosis of the skin;4~6 weeks after the operation, the internal fixation of Kirschner needle was extracted;9 of them were followed up for 6 ~24 months. The finger function recovered well, the phalanx and the ulnar flap healed well;the color of the flap was close to the normal hand finger skin;the sensory recovery was good;forearm rotation was good. There was no influence on rotation, weight and wrist flexion and extension. Conclusion The free ulnar artery carpal branch flap with ulnar flap is a convenient and effective method to repair the defect of the finger composite tissue. It has the advantages of constant skin branch, no damage to the trunk blood vessel, rapid healing of bone tissue, convenient and flexible cutting, and good appearance of finger.
