摘 要:目的:研究人参配伍白术前后皂苷成分变化及治疗慢性萎缩性胃炎的疗效。方法:采用HPLC法测定人参配伍白术前后皂苷化学成分的变化;将大鼠随机分为空白组、模型组、配伍前皂苷组和配伍后皂苷组,用主动免疫法复制慢性萎缩性胃炎模型。治疗结束后观察各组胃黏膜组织病理学及超微结构改变。结果:经HPLC法测定,与配伍前皂苷组相比,配伍后皂苷组中7种皂苷含量均上升,并出现1个新的色谱峰。与正常组相比,模型组胃组织病理评分明显改变;与模型组相比,配伍前皂苷60.3mg/kg组及配伍后皂苷132.6mg/kg组胃组织病理评分均显著降低。电镜观察表明模型组大鼠胃黏膜出现皱襞隆起,胃小凹变形,小凹壁上皮细胞萎缩等,各给药组与模型组相比较,胃小凹形状大小趋向规则,配伍后皂苷组萎缩程度轻于配伍前皂苷组。结论:人参皂苷可有效治疗慢性萎缩性胃炎大鼠胃黏膜损伤,配伍白术后效果更佳;可能与人参皂苷含量增加并有新成分生成有关。Objective: To compare the constituents and the effect on chronic atrophic gastritis of total ginsenosides of Ginseng Radix( GR) before and after combined with Rhizoma Atractylodis Macrocephalae( AMR). Methods: The chemical composition changes were studied by HPLC.Rats were randomly divided into the control group,the model group,ginsenoside group and combined ginsenoside group. The chronic atrophic gastritis model was established by the active immunization. At the end of treatment,the pathological and ultrastructural changes of gastric mucosa were observed. Results: HPLC analysis suggested that the contents of seven ginsenoside were increased after combined with AMR,and a new chromatographic peak appeared. Compared with the control group,the pathological scores of gastric tissues in the model group were significantly changed. Compared with the model group,the gastric histopathological scores in ginsenoside group and the combined ginsenoside group were significantly changed. The histopathological score in combined ginsenoside group was lower than that in ginsenoside group. Electronic microscopy result showed that plica was formed in the gastric mucosa of model rats,foveola gastrica was deformed,and the epithelial cells of the small concave wall were atrophied. Compared with the model group,the size of the foveola gastrica tended to be regular,and slighter atrophy was observed in combined ginsenoside group than that in the ginsenoside group. Conclusion: Ginsenosides can effectively treat gastric mucosal injury in rats with chronic atrophic gastritis,and shows better effect when combined with AMR. It may be related to the higher content of ginsenoside and the new chromatographic peak. The experimental results provide a basis for clarifying the scientific connotation of mutual promotion.
