盐酸坦索罗辛缓释胶囊联合热淋清颗粒在输尿管结石合并膀胱刺激征患者中的临床观察

摘　　要：目的:采用前瞻性随机对照临床试验,评估盐酸坦索罗辛缓释胶囊联合热淋清颗粒治疗输尿管膀胱壁段结石合并膀胱刺激征患者的临床疗效。方法:103例输尿管膀胱壁段结石合并膀胱刺激征的患者随机分为热淋清颗粒组、盐酸坦索罗辛缓释胶囊组和联合用药组,分别给予热淋清颗粒、盐酸坦索罗辛缓释胶囊和两药联合治疗,疗程均为2周。记录患者治疗前后尿急、尿痛程度、结石的排出时间、排出率以及药物不良反应发生情况。结果:与盐酸坦索罗辛缓释胶囊组相比,热淋清颗粒组、联合用药组的结石排出率较高,结石排出时间较短(P<0.05)。治疗3 d和7 d后,联合用药组、盐酸坦索罗辛缓释胶囊组的的USS及VAS评分显著低于热淋清颗粒组(P<0.05)。盐酸坦索罗辛缓释胶囊组患者口干发生率及不良反应发生率显著高于热淋清颗粒组和联合用药组(P<0.05),联合用药组不良反应发生率显著高于热淋清颗粒组(P<0.05)。结论:盐酸坦索罗辛缓释胶囊联合热淋清颗粒治疗能够加速输尿管膀胱壁段结石的排出,并有效缓解患者的膀胱刺激症状,改善患者生活质量。Objective: To evaluate the clinical efficacy of tamsulosin sustained-release capsules combined with Relinqing granule in patients with ureteral calculi complicated with bladder irritation using a randomized,controlled and prospective clinical trial. Methods: 103 patients with ureteral bladder wall stones complicated with bladder irritation syndrome were randomly divided into Relinqing Granule group,Tamsulosin Hydrochloride Sustained Release Capsule group and combined drug group. Relinqing Granule and Tamsulosin Hydrochloride Sustained Release Capsule were given respectively for two weeks. Urinary urgency,degree of urinary pain,time of stone excretion,excretion rate and adverse drug reactions were recorded before and after treatment. Results: Compared with that in tamsulosin hydrochloride sustained-release capsules group,the calculus discharge rates in Relinqing granule group and the combination group were higher with shorter average stone discharge time,and the differences were statistically significant( P < 0. 05). After the 3-day and 7-day treatment,the scores of USS and VAS in the combination group and tamsulosin hydrochloride sustained-release capsules group were lower than those in Relinqing granule group,and the differences were statistically significant( P < 0. 05). The incidence of dry mouth and adverse reactions in tamsulosin hydrochloride sustained-release capsules group was evidently higher than that in Relinqing granule group and the combination group,and the incidence of adverse drug reactions in the combination group was significantly higher than that in Relinqing granule group( P < 0. 05). Conclusion: Tamsulosin hydrochloride sustained-release capsules combined with Relinqing granule can accelerate the expulsion of intramural ureteral stones,effectively relieve the vesical irritability and improve the quality of life of patients.
