摘 要:目的探讨精子核蛋白组型转换与精液参数、精子DNA损伤及复发性流产的相关性。方法研究对象为64例复发性流产患者配偶,对其进行精液常规、精子核蛋白组型转换和DNA碎片指数(DFI)检测。以核蛋白组型转换异常率30%为界分为正常组和异常组,应用SPSS20.0进行独立样本t检验分析年龄、精液参数(精液量、精子浓度、前向运动精子百分比和正常形态百分率)、DFI及流产次数的差异;采用Pearson法分析核蛋白组型转换与年龄、精液参数、DFI及流产次数的相关性。结果核蛋白组型转换正常组(异常率>30%,10例)与核蛋白组型转换异常组(异常率〉30%,54例)相比,精子浓度、正常形态百分率与DFI两组之间差异有统计学意义(P<0.01),而年龄、精液量、前向运动精子百分比和流产次数则无统计学差异(P>0.05).Pearson相关分析结果,精子核蛋白组型转换异常率与精子DFI呈显著正相关(P<0.01),与年龄、精液参数及流产次数则无明显相关性(P>0.05)。结论在复发性流产患者配偶中,核蛋白组型转换正常组和异常组在精子浓度、正常形态百分率以及DFI存在显著差异,且核蛋白组型转换异常率与精子DNA损伤呈显著正相关,可以作为复发性流产患者的评估指标。Objective To investigate the correlation between sperm nucleoprotein transformation and semen parameters, sperm DNA damage, recurrent abortion. Methods We collected semen samples :from 64 patients with recurrent abortion, whose semen routine, sperm nucleoprotein transformation and DNA fragment index were detected. According to whether the nucleoprotein transformation abnormal rate is 30%, it is divided into two groups. The age, semen parameters (semen volume, sperm concentration, forward motility sperm percentage and normal morphology percentage), DFI and abortion times were analyzed by SPSS 20.0 t-test. Pearson method was used to analyze the correlation between nucleoprotein transformation and age, semen parameters, DFI and the number of miscarriages. The definition P<0.05 has statistical significance. Results There were significant differences in sperm concentration, percentage of normal morphology and DFI between normal group (abnormal rate is 30% in 10 cases) and abnormal group (abnormal rate >30% in 54 cases)(P<0.01). However, there was no significant difference in age, sperm volume, percentage of forward motility sperm and t abortion times (P>0.05). Pearson correlation analysis showed that there was a significant positive correlation between spemi nucleoprotein transfonnation abnormal and DFI (r =0.820, P<0.01), but no significant correlation with age, semen parameters and abortion times (P>0.05). Conclusions There are significant differences in sperm concentration, percentage of normal morphology and DFI between normal group and abnormal group of sperm nucleoprotein transfonnation in patients with recurrent abortion, which is closely related to sperm DNA damage and can be used as an evaluation index for recun'ent abortion.
