摘 要:目的比较局麻下电磁导航支气管镜与普通支气管镜行肺部病变活检的成功率。方法回顾性分析2017年3月-2018年5月我中心进行的40例电磁导航支气管镜下肺部病变活检患者以及同期40例经局麻下普通支气管镜下肺部病变活检患者围术期的临床资料。结果纳入病例中电磁导航组男性25例,平均年龄(58.21±15.67)岁;普通支气管镜组男性20例,平均年龄(58.43±8.44)岁,两组性别、年龄差异均无统计学意义(P> 0.05)。电磁导航组肺部病变直径为(2.43±0.67) cm,普通支气管镜组肺部病变直径为(4.00±1.83) cm,差异有统计学意义(P <0.001)。局麻下磁导航组活检成功率为92.5%,普通支气管镜组活检成功率为75.0%,差异有统计学意义(P=0.034)。两组均无药物不良反应以及相关并发症。结论与局麻下普通支气管镜相比,局麻下电磁导航支气管镜活检成功率更高,可诊断的肺部病变直径更小,更靠外周,范围更广。Objective To compare the success rate of electromagnetic navigation bronchoscopy and conventional bronchoscopy for lung biopsy under local anesthesia. Methods Clinical data about 40 patients underwent lung lesions biopsy by electromagnetic navigation bronchoscope and 40 patients with lung lesions biopsy by conventional bronchoscope from March 2017 to May 2018 in the First Medical Center of Chinese PLA General Hospital were retrospectively analyzed. Results There was no significant difference in gender and age between two groups(male, 25 vs 20;age, 58.21±15.67 years vs 58.43±8.44 years, P > 0.05,respectively). The diameter of pulmonary lesions in electromagnetic navigation group was(2.43±0.67) cm, and it was(4.00±1.83)cm in conventional bronchoscopy group with signi?cant difference(P < 0.001). Under local anesthesia, the success rate of biopsy was 92.5% in electromagnetic navigation group, signi?cantly higher than 75% in conventional bronchoscopy group(P=0.034). There was no adverse drug reactions and related complications in two groups. Conclusion Compared with conventional bronchoscopy,the success rate of electromagnetic navigation bronchoscopy under local anesthesia is higher. It is more effective in biopsy to lesions with small diameter, and close to the peripheral part of the lung.
