摘　　要：目的:探讨单侧椎弓根螺钉固定并椎间融合治疗极外侧老年腰椎间盘突出症的临床效果.方法:选择我院治疗的老年极外侧腰椎间盘突出患者42例,每组各21例,对照组给予患者保守治疗,观察组采用单侧椎弓根螺钉固定并椎间融合治疗,观察2组患者临床治疗效果.结果:2组患者均随访1年后腰痛和腿痛的视觉模拟评分均较治疗前改善,但观察组改善较对照组明显,经统计学分析比较,差异有统计学意义(P<0.05).观察组ODI功能障碍指数评分改善情况优于对照组,经统计学分析比较,差异有统计学意义(P<0.05).结论:采用单侧椎弓根螺钉固定并椎间融合治疗极外侧老年腰椎间盘突出症具有创伤小,恢复快,住院时间短等优点,能够有效改善患者的功能障碍情况,值得在临床上大力推广使用.Objective: To evaluate the clinical effect of unilateral pedicle screw fixation and intervertebral fusion using PEEK for the treatment of extremely lateral elderly lumbar intervertebral disc herniation. Methods: 42 patients with protrusion of the lateral lumbar intervertebral disc were elected in our hospital, and were divided into two groups by different treatments: the treatment group and the control group (21 persons per group);The patients were treated by non - operation in control group and the others were implemented by unilateral pedicle screw fixation and intervertebral fusion;Then, the effect of treatment were observed and evaluated between two groups. Results: Follow - up one year, the visual analogue scores( VAS) and ODI of lower back pain and ischialgia were improved effectively in two groups than before treatments, but the effect of treatment group were better than control group. The significant differences were found in two groups(P <0. 05). Conclusion: Unilateral pedicle screw fixation and PEEK intervertebral fusion to treat the extremely lateral lumbar disc herniation has the advantages of small trauma, quick recovery and short hospital stay. It can effectively improve the patients-functional disorder, and it is worthy to be widely used in clinical practice.
