加减温胆汤与奥美拉唑联合治疗咽喉反流性疾病的疗效

摘　　要：目的研究加减温胆汤与奥美拉唑联合治疗咽喉反流性疾病的疗效。方法选取2011年12月至2016年7月就诊于晋中市第一人民医院门诊的咽喉反流性疾病患者180例,随机分为研究组和对照组,每组90例。对照组患者口服奥美拉唑20mg,每日2次;口服吗丁啉10mg,每日3次。研究组患者在奥美拉唑(用法用量同上)用药基础上,联合运用加减温胆汤,每日1剂,水煎服,早晚分2次服用。连续用药8周,观察临床疗效,治疗结束后统计总有效,继续随访,分别于治疗后第4、8、12个月对随访患者进行反流症状指数评分量表(RSI)症状评分及填写反流体征评分量表(RFS),记录复发情况。结果研究组显效47例,有效39例,无效4例,总有效率为95.56%。对照组显效35例,有效38例,无效17例,总有效率为81.11%。研究组疗效优于对照组(P<0.01)。随访1年,研究组有效患者86例中,复发24例;对照组有效患者61例中,复发27例,研究组复发率低于对照组(P<0.05)。结论加减温胆汤与奥美拉唑联合治疗咽喉反流性疾病的疗效优于单纯口服西药,中西医联合治疗(加减温胆汤与奥美拉唑)能显著降低患者的远期复发率。Objective To study the effects of modified wendan decoction combined with omeprazole on laryngopharyngeal reflux disease(LPRD).Methods From December 2011 to July 2016,180 patients with LPRD in the First People's Hospital of Jinzhong were randomly divided into study group and control group,with 90 cases in each group.The patients in control group orally took omeprazole(20 mg each time,2 times per day)and domperidone(10 mg each time,3 times per day),and the patients in study group took omeprazole as the same as above,and took modified wendan decoction in addition,decocted in water,took twice daily in the morning and evening.Patients kept continuous medication for 8 weeks to observe the clinical efficacy.After treatment,the total effective cases were counted and followed up.The reflux symptom index(RSI)symptom scores and reflux finding score(RFS)scales were evaluated in the 4th,8th and 12th month respectively to record the recurrence.Results In the study group,treatments were apparently effective in 47 cases,effective in 39 cases,invalid in 4 cases,and the total effective rate was 95.56%.In the control group,apparently effective in 35 cases,effective in 38 cases,invalid in 17 cases,and the total effective rate was 81.11%.The effect in the study group was better than that in the control group(P<0.01).After one-year follow-up,24 cases of the 86 patients in the study group recurred,while 27 cases of the 61 patients in the control group recurred.The recurrence rate in the study group was lower than that in the control group(P<0.05).Conclusions The effects of modified wendan decoction combined with omeprazole in the treatment for laryngopharyngeal reflux disease are much better than that of simply oral western medicine.It can significantly reduce the forward recurrence rate.
