摘 要:目的分析高通量血液透析与常规血液透析治疗慢性尿毒症的效果。方法选取2016年6月至2018年6月大同市第五人民医院收治的80例慢性尿毒症患者为研究对象,采取随机数字表法分为对照组与观察组,每组40例。对照组采取常规血液透析治疗;观察组采取高通量血液透析治疗。比较两组患者血液透析前后的相关指标及两组患者感染、低血压等不良反应发生情况。结果治疗后,两组血尿素氮(BUN)、血肌酐(Scr)、血钾等指标比较差异未见统计学意义(P>0.05);观察组血清甲状旁腺激素(IPTH)、β2-微球蛋白(β2-MG)等指标低于对照组,血红蛋白(Hb)水平高于对照组,差异有统计学意义(P<0.05);对照组透析治疗期间不良反应发生率(25.0%)高于观察组(7.5%),差异有统计学意义(P<0.05)。结论与常规血液透析相比,高通量血液透析对中大分子毒素的清除效果更好,且能够减少透析相关并发症发生,是治疗慢性尿毒症的有效手段。Objective To analyse the effects of high-flux hemodialysis and routine hemodialysis on patients with chronic uremia. Methods Eighty patients with chronic uremia who were treated in the Fifth People’s Hospital of Datong from June 2016 to June 2018 were enrolled in the study. The patients were divided into control group and observation group by random number table method, with 40 cases in each group. Patients in control group were treated by routine hemodialysis, while patients in observation group were treated by high-flux hemodialysis. The related indexes of the two groups were compared before and after hemodialysis, and incidences of complications were compared between the two group, such as infection and hypotension. Results After treatment, there was no significant difference in blood urea nitrogen (BUN), serum creatinine (Scr), K+ or other indexes between the two groups (P>0.05). The parathyroid hormone (IPTH) and β2-microglobulin (β2-MG) of observation group were significantly lower than those of control group, and hemoglobin (Hb) level was higher than that of control group (P<0.05). The incidence of adverse reactions during hemodialysis in control group (25.0%) was higher than that in observation group (7.5%), and the difference was significant (P<0.05). Conclusions Compared with conventional hemodialysis, high-flux hemodialysis has better scavenging effects on macromolecular toxins, and it can reduce dialysis-related complications, which is an effective treatment for chronic uremia.
