摘  要:目的探讨布地奈德气雾剂治疗成人晚发型哮喘的效果及对患者痰液中炎症因子、嗜酸性粒细胞(EOS)及中性粒细胞计数(NEU)水平的影响。方法选取2017年4月至2018年4月晋中市第二人民医院收治的成人晚发型哮喘患者100例作为研究对象,其中成人晚发型轻中度哮喘患者50例纳入A组,重度哮喘患者50例纳入B组,另选取同期健康体检者50例纳入C组。100例患者根据病情给予布地奈德气雾剂治疗,分别采集患者治疗前及激素治疗后的诱导痰液,检测痰液中炎症因子浓度、EOS及NEU水平。结果A组4例嗜酸细胞性哮喘及3例混合性哮喘转变为寡细胞性哮喘;B组2例嗜酸细胞性哮喘转变为混合性哮喘,2例嗜酸细胞性哮喘转变为中性粒细胞性哮喘。治疗前后,A、B两组气道炎症分型比较差异未见统计学意义(P>0.05)。治疗前,A、B组气道内EOS比例高于C组,A组NEU低于C组,B组NEU高于A、C组,差异有统计学意义(P<0.05);治疗后,A组EOS、NEU水平低于治疗前,B组EOS水平低于治疗前且NEU水平高于治疗前,差异有统计学意义(P<0.05)。治疗前,A、B组气道白细胞介素-17(IL-17)、白细胞介素-5(IL-5)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)水平均高于C组,差异有统计学意义(P<0.05);治疗后,A、B组IL-17、IL-5、IL-6、IL-8水平降低,差异有统计学意义(P<0.05)。结论激素对不同严重程度的成人晚发型哮喘患者痰液中炎症因子、EOS及NEU水平有一定的调节作用,对以后临床治疗成人晚发型哮喘有一定的指导作用。Objective To investigate the effects of budesonide aerosol on late-onset asthma in adults and its effects on inflammatory factors, eosinophils (EOS) and neutrophils count (NEU) in sputum. Methods One hundred adult late-onset asthma patients in the Second People’s Hospital of Jinzhong from April 2017 to April 2018 were selected as the subjects. Of them, 50 patients with late-onset mild to moderate asthma were included in group A, and 50 patients with severe asthma were included in group B. Another 50 healthy persons in the same period were selected and included in group C. One hundred patients were treated with budesonide aerosol according to the condition of the disease. Induced sputum was collected before treatment and after hormone treatment, and the levels of inflammatory factors, EOS and NEU in sputum were detected. Results In group A, 4 cases of eosinophilic asthma and 3 cases of mixed asthma were transformed into oligotropic asthma. In group B, 2 cases of eosinophilic asthma were transformed into mixed asthma and 2 cases of eosinophilic asthma were changed to neutrophil asthma. Before and after treatment, there was no significant difference in airway inflammation types between group A and group B (P>0.05). Before treatment, the proportion of EOS in airway in group A and group B was higher than that in group C;NEU in group A was lower than that in group C;and NEU in group B was higher than that in group A and group C (P<0.05). After treatment, the levels of EOS and NEU in group A were lower than those before treatment, while EOS was lower and the level of NEU was higher in group B than that before treatment (P<0.05). Before treatment, the levels of airway interleukin-17 (IL-17), interleukin-5 (IL-5), interleukin-6 (IL-6), interleukin-8 (IL-8) in group A and group B were higher than those in group C (P<0.05). After treatment, the levels of IL-17, IL-5, IL-6 and IL-8 in group A and group B decreased, and the difference was significant (P<0.05).
