家槐鸡子黄油膏联合美皮康治疗老年髋部骨折后Ⅲ期褥疮的临床疗效

摘　　要：目的观察家槐鸡子黄油膏涂擦联合美皮康贴敷治疗老年髋部骨折后Ⅲ期褥疮的疗效。方法2011年1月-2015年12月河南省洛阳正骨医院髋部治疗中心收治的老年髋部骨折171例225处Ⅲ期褥疮,根据治疗方法分为观察组40例53处褥疮、治疗组58例79处褥疮和对照组73例93处褥疮。在集束化护理支持下,观察组家槐鸡子黄油膏擦涂联合美皮康贴敷创面治疗,治疗组美皮康贴敷创面治疗,对照组凡士林油纱贴敷治疗。以褥疮创面面积的增减变化、创面肉芽组织生长状况、创面渗出情况为观察指标,治疗2周后比较3组褥疮痊愈病例的治愈时间及3组的总体疗效。结果观察组38处褥疮痊愈,最短痊愈时间6 d,平均(7. 8±1. 5) d,治疗组24处褥疮痊愈,最短痊愈时间8 d,平均(10. 3±2. 3) d,对照组8处褥疮痊愈,最短痊愈时间12 d,平均(13. 1±4. 8) d,3组褥疮痊愈时间比较差异有统计学意义(P <0. 05)。观察组治愈率和有效率与治疗组和对照组比较,差异有统计学意义(P <0. 05)。结论家槐鸡子黄油膏联合美皮康贴敷治疗褥疮,为创面愈合补充了外源物质、提供了理想的湿性愈合环境,可促进褥疮创面快速愈合。老年髋部骨折后Ⅲ期褥疮可优先选择应用家槐鸡子黄油膏擦涂联合美皮康贴敷治疗。Objective To observe the effects of jiahuai jizi huangyou ointment and mepilex on pressure sore at the third stage in aged hip fracture.Methods From January2011 to December2015,171 patients with225 pressure sores of the third stage in aged hip fracture were collected from the Hip Department of Henan Luoyang Bonesetting Hospital.According to the treatment methods,they were divided into three groups,named the treatment group(73 cases,93 pressure sores),the No.1 control group(58 cases and 79 pressure sores)and the No.2 control group(40 cases,53 pressure sores).Under the support of cluster nursing care,the external application with jiahuai jizi huangyou ointment and mepilex on the wound was used in the treatment group.In the control No.1 group,mepilex plaster was used and in the control No.2 group,petrolatum oil gauze plaster was used on the wound.The wound area,the growth of granulation tissue on the wound and wound exudation were taken as the observation indicators.After2-week treatment,the healing time and the total effect were compared among the three groups.Results In the treatment group,38 sores were cured and the shortest healing time was 6 days,at the average of(7.8±1.5)days.In the control No.1 group,24 sores were cured and the shortest healing time was 8 days,at the average of(10.3±2.3)days.In the control No.2 group,8 sores were cured and the shortest healing time was12 days,at the average of(13.1±4.8)days.In comparison of healing time,the differences were significant statistically among the three groups(P<0.05).The curative rate was40.9% and the effective rate 92.5% in the treatment group.They were30.4% and 88.6% in the control No.1 group and were15.1% and 79.3% in the control No.2 group successively.Compared with the control No.1 and No.2 groups,the differences were significant statistically in the treatment group(P<0.05).Conclusion The combined treatment with jiahuai jizi huangyou ointment and mepilex provides the external-source substance to fill up the wound and establishes the satisfactory humid hea
