联合确定性纤维追踪技术和注意网络测试研究海马硬化型癫痫结构和执行功能的结果分析

摘　　要：目的:联合确定性纤维追踪技术和注意网络测试(attentional network test,ANT)探讨海马硬化型癫痫患者脑白质结构损害与执行功能的关系。方法:对43例海马硬化性癫痫患者及42例性别、年龄、受教育年限相匹配的健康对照者进行ANT和DTI扫描,采用PANDA软件及确定性纤维追踪技术分析DTI数据,以及E-Studio软件计算执行反应时间(reaction time,RT)。结果:与健康对照组相比,患者组双侧额上回、双侧侧扣带回及颞中回各向异性值(fractional anisotropy,FA)、纤维数量(fiber number,FN)明显低(P<0.05),执行RT明显长(P<0.05);患者组右侧额上回FA值与执行RT及FN值与执行RT均呈负相关(r=-0.36,P=0.018;r=-0.309,P=0.044)。结论:海马硬化型癫痫患者白质结构和执行功能均受损,其右侧额上回FA、FN值与执行RT呈负相关。Objective:To investigate the relationship between the white matter deficits and the execution function of hippocampal sclerotic epilepsy patients by using deterministic tractography and attentional network test(ANT).Method:43 patients with hippocampal sclerotic epilepsy and 42 healthy controls with gender,age and years of education were examined by ANT and DTI.PANDA software and deterministic fiber tracing technique were used to analyze DTI data,and E-Studio software was employed to compute executive reaction time(RT).Result:Compared with the healthy control group,anisotropy(FA),fibrous quantity(FN) of the bilateral superior frontal gyrus,bilateral bilateral cingulate gyrus and middle temporal gyrus in the patients group were significantly decreased(P<0.05), and the executive reaction time was significantly prolonged(P<0.05);the FA and FN values on the right frontal were negatively correlated with the response time(r=-0.36,P=0.018.r=-0.309,P=0.044).Conclusion:The white matter structure and executive function of hippocampal sclerotic epilepsy are impaired,and the FA and FN values on the right frontal are negatively correlated with the response time.
