NOSES技术治疗结直肠癌患者的术后观察与护理

摘　　要：目的探讨经自然腔道取标本手术(NOSES)治疗结直肠肿瘤患者的术后观察与相关护理要点。方法术前对患者进行心理疏导及饮食指导,术中保温,术后使用镇痛泵并进行疼痛评估和干预,早期下床活动、早期进食,肛周皮肤护理,导管护理等。结果平均手术时间(152.5±40.5)min,平均术中出血量(38.3±32.5)mL,下床活动时间(1.5±0.4)d。平均术后排气时间(3±1.1)d。进食流质时间(2.3±0.8)d。术后平均住院时间(11±2.3)d。结论对NOSES结直肠肿瘤手术患者围手术期进行心理护理、术后镇痛、早期活动、早期饮食等护理措施是安全可行的,并且可以促进患者早期康复、缩短住院时间、优化医疗资源分配。Objective To investigate the postoperative observation and related nursing points of patients undergoing colorectal surgery through natural cavity sampling surgery(NOSES).Methods Preoperative psychological counseling and diet guidance,intraoperative heat preservation,postoperative analgesia pump and pain assessment and intervention,early bed activity,early feeding,perianal skin care,catheter care and so on.Results The average operative time was(152.5±40.5)min,and the average intraoperative blood loss was(38.3±32.5)mL,and the activity time of the lower bed was(1.5±0.4)d.The average time of postoperative exhaust was(3±1.1)d.The feeding time was(2.3±0.8)d.The average hospitalization time after operation was(11±2.3)d.Complications Psychological nursing,postoperative analgesia,early activity and early diet are safe and feasible for the perioperative period of NOSES colorectal tumor operation,and it can promote the early rehabilitation,shorten the time of hospitalization and optimize the distribution of medical resources.
