摘　　要：目的探讨新生儿缺氧缺血性脑损伤(HIBD)的MRDWI与新生儿神经行为测定(NBNA)的相关性方法选取临床确诊的新生儿HIBD患者78例.所有患儿7d内行颅脑MR平扫及DWI检查.按常规序列扫描结果分为轻、中、重3组.同时获取所有研究对象的7个ROK分别为额叶深部白质.顶叶深部白质,半卵圆中心.内蟆后肢,丘脑.脑干,小脑甘质)的AD值,并对各组AIX'值与NBNA评分进行相关性研究。结果78例患儿分轻度组26例,中度组28例.重度组24例.除小脑白质外.3组患儿同一部位ADC值随着病情严重程度减低,差异有统计学意义(PV0.05);除小脑白质外.患儿各ROIADC值与NBNA评分呈正相关(P<0.01)。结论新生儿HIBD的DWI表现(用ADC值表达)与NBNA评分呈正相关.NBNA评分是已证实的客观指标.故DWI序列可为缺氧缺血性脑病提供早期客观的诊断依据。Objective To explore the correlation of neonatal behavioral neurological assessment ( NBNA) and DWI in neonate with hypoxic ischemic brain damage ( HIBD(.Methods A total of 78 patients diagnosed with HIBD were collected. All patients underwent brain conventional MR and DWI examination within 7 days. According to the result of conventional MRI, the patients were assigned into three groups: the mild , moderate, and serve groups. Meanwhile, the AIX' in seven R()1, including the deep while matter of frontal lobe,deep white matter of parietal lobe, centrum ovale majus, posterior limb of the internal capsule, thalamus, brainstem, and white matter of cerebellum for all the palienis were obtained. I'he correlation of AfX,values and NBNA scores for each group were investigated.Results In 78 patients with HIBD, there were 26,28,24 patients in the mild, moderate, a nd severe group, respectively. The ADC values in lhe same region of patients for three groups decreased with the disease severity (P<0.05). Except for the white matter of cerebellum, the ADC values in the each interest regions of patients were positively correlated with the NBNA scores ( P <0.01 ).Conclusion DWI (expressed by the AIX' values) of neonate with HIBD is positively correlated with the NBNA scores.The NBNA scores have been proved to be an objective indicator, thus, the DWI can provide early diagnostic basis for hypoxic-isschemic encephalopathy.
