血小板抗体检测在新生儿血小板减少症诊治中的应用

摘　　要：目的:探讨检测血小板相关抗体(PAIg)的表达水平在新生儿血小板减少症诊断及治疗过程中的应用价值。方法:选择2015-01~2017-12我院收治的60例血小板减少症新生儿作为患儿组,同期选择60例健康新生儿作为对照组,采用流式细胞术检测所有新生儿的外周血PAIg的表达水平,分析PAIg对新生儿血小板减少症的诊断价值;新生儿血小板减少症患儿采用血小板输注治疗并在治疗后检测PAIg的表达水平,分析PAIg对治疗效果的影响。结果:新生儿血小板减少症患儿的PAIgA、PAIgM及PAIgG表达水平均明显高于对照组(P<0.05);重症组患儿的PAIgA、PAIgM及PAIgG表达水平均明显高于轻度组患儿(P<0.05);经治疗后42例(70.00%)患儿有效,18例(30.00%)患儿无效;治疗前,有效组患儿的PAIgM、PAIgG表达水平均明显高于有效组患儿(P<0.05);有效组患儿的PAIgA与对照组无明显差异(P>0.05);治疗后,有效组患儿的PAIgA、PAIgM及PAIgG表达水平均明显低于治疗前(P<0.05);而无效组治疗前后PAIgA、PAIgM及PAIgG表达水平无明显差异(P>0.05)。结论:PAIg的表达水平与新生儿血小板减少症病情严重程度密切相关,是重要的诊断依据,同时PAIgM和PAIgG的表达水平与血小板输注的疗效密切相关,是判定治疗效果的重要指标。Objective:To explore the value of detecting the expression level of platelet associated antibody(PAIg)in the diagnosis and treatment of neonatal thrombocytopenia.Methods:60 newborns with thrombocytopenia in our hospital in December-2017 January 2015 were selected as children,and 60 healthy newborns were selected as control group at the same time.Flow cytometry was used to detect the expression level of peripheral blood PAIg in all newborns,and the diagnostic value of PAIg for neonatal blood plate reduction was analyzed.Thrombocytopenia children were treated with platelet transfusion,and the expression level of PAIg was detected after treatment.The effect of PAIg on the therapeutic effect was analyzed.Results:The expression level of PAIgA,PAIgM and PAIgG in children with neonatal thrombocytopenia was significantly higher than that in the control group( P < 0.05 ).The expression level of PAIgA,PAIgM and PAIgG in the severe group was significantly higher than that in the mild group( P < 0.05 );42 cases(70%)were effective and 18 cases(30%)were not effective.The expression level of PAIgM and PAIgG in the effective group was significantly higher than that in the effective group( P < 0.05 ),and there was no significant difference between the effective group and the control group( P > 0.05 ).After the treatment,the expression level of PAIgA,PAIgM and PAIgG in the effective group was significantly lower than that before the treatment( P < 0.05 ),and the PAIgA,PAIgM and P before and after the treatment were not effective.There was no significant difference in the expression level of AIgG( P > 0.05 ).Conclusion:The expression level of PAIg is closely related to the severity of neonatal thrombocytopenia,and it is an important diagnostic basis.At the same time,the expression of PAIg M and PAIg G is closely related to the efficacy of platelet transfusion,which is an important index to determine the effect of the treatment.
