基于升降学说探讨糖尿病肾病的治疗

摘　　要：糖尿病肾病是1型及2型糖尿病最常见的微血管并发症之一,也是导致终末期肾病的主要原因。目前,西医治疗仍难以从根本上改变糖尿病肾病的进程,研究发现中医药辨证治疗糖尿病肾病具有显著的疗效,尤其与西医联合疗效更为显著。升降学说是中医理论体系中的重要部分,调整气机升降复常是中医治疗疾病的基本治则之一,本文通过梳理历代医家对糖尿病肾病的相关论述,探讨糖尿病肾病的演变规律与升降学说的关系,总结现代医家应用升降学说论治糖尿病肾病的经验。Diabetic nephropathy (DN) is the most common microvascular complication in Type 1 and 2 diabetes mellitus, which is also the main cause for end stage renal failure. At present, western medicine treatment is still difficult to fundamentally change the process of DN. Studies have found that traditional Chinese medicine (TCM) treatment of diabetic nephropathy based on syndrome differentiation has significant effect, especially in combination with western medicine. The theory of Rise and Fall is an important part of the theoretical system of TCM. Regulating the ascending and descending of Qi is one of the basic principles of treating diseases in TCM. This article discussed the relationship between the development of DN and the Rise and Fall theory through collating related discourse of DN by ancient TCM physicians, as well as summarizing the clinical experiences of present TCM physicians on DN using the Rise and Fall theory.
