摘  要:目的探讨PKP手术中应用明胶海绵辅助利多卡因治疗胸腰椎骨折的临床疗效。方法将40例骨质疏松性胸腰椎压缩骨折患者随机分为两组,观察组采用PKP术中明胶海绵辅助利多卡因治疗,对照组采用传统局部麻醉下行PKP手术治疗,比较两组不同时间点的VAS疼痛评分、Cobb角及椎体高度。结果观察组术中的VAS评分显著高于对照组,差异有统计学意义(P<0.05),术后两组之间的VAS评分差异无统计学意义(P>0.05)。两组患者各时间点的椎体高度和Cobb角差异无统计学意义(P>0.05)。结论与传统PKP手术相比,PKP术中结合明胶海绵辅助利多卡因治疗胸腰椎骨折,可有效减轻术中疼痛,且对术后椎体高度、后凸畸形的矫正效果无不良影响。Objective To investigate the clinical effect of gelatin sponge assisted lidocaine in the treatment of thoracolumbar fracture in PKP. Methods 40 patients with osteoporotic thoracolumbar vertebral compression fractures were randomly divided into two groups. The observation group was treated with gelatin sponge assisted lidocaine during PKP, and the control group was treated with PKP under traditional local anesthesia. VAS pain score, Cobb angle and vertebral height at different time points were compared between the two groups. Results The VAS score in the observation group was significantly higher than that in the control group (P< 0.05). There was no significant difference in the VAS score between the two groups (P>0.05). There was no significant difference in vertebral height and OBB angle between the two groups at each time point (P>0.05). Conclusion Compared with the traditional PKP operation, the combination of gelatin sponge and lidocaine in the treatment of thoracolumbar fracture can effectively relieve intraoperative pain, and has no adverse effect on the correction of vertebral height and kyphosis deformity.
