改良Fox术及改良眼轮匝肌折叠术治疗老年下睑内翻的效果研究

摘　　要：目的:对比改良外眦部梭形皮肤切除术(Fox术)及改良眼轮匝肌折叠术(Wheeler术)对老年下睑内翻的治疗效果。方法:将72例下睑内翻患者分为观察组(n=36,43眼)和对照组(n=36,47眼)。观察组采用改良Fox术,对照组采用改良Wheeler术。比较两组患者满意度、复发率、疗效和并发症发生情况。结果:两组近期满意度评分及近期复发率比较[(8.14±1.45)分、4.65%vs(8.17±1.41)分、2.13%],差异无统计学意义(P>0.05)。观察组远期满意度评分(7.71±1.23)分及远期复发率(9.30%)明显优于对照组[(6.57±1.12)分,29.79%],差异具有统计学意义(P<0.05)。观察组并发症发生率为6.98%,对照组为8.51%,观察组总有效率为93.02%,对照组为89.36%,组间比较差异均无统计学意义(P>0.05)。结论:在下睑内翻的手术治疗中,改良Fox术与改良Wheeler术近期疗效相近,但改良Fox术的远期复发率优于改良Wheeler术,患者满意度更高。Objective To compare the therapeutic effects of improved fusiform skin resection of outer canthus ( Fox operation) and improved orbicularis oculi muscle folding (Wheeler operation) on lower eyelid entropion. Methods 72 patients with lower eyelid entropion were divided into the observation group (36 cases, 43 eyes ) and the control group (36 cases, 47 eyes). The observation group was treated with improved Fox operation and the control group with improved Wheeler operation. The satisfaction, the rate of recurrence, adverse reactions and efficacy were compared between the two groups. Results There was no significant difference in short-term satisfaction score and recurrence rate between the two groups [(8.14±1.45) score, 4.65% vs (8.17±1.41) score, 2.13%, P >0.05]. The long-term satisfaction score (7.71±1.23) score and the long-term recurrence rate (9.30%) of the observation group were significantly better than those of the control group [(6.57±1.12) score, 29.79%], the differences were statistically significant(P <0.05). The incidence of complications was 6.98% in the observation group and 8.51% in the control group, the total effective rate was 93.02% in the observation group and 89.36% in the control group, there were no significant difference between the two groups(P >0.05). Conclusion In the operation of lower eyelid entropion, the shortterm therapeutic effect of improved Fox operation is similar to that of improved Wheeler operation, but the rate of long-term recurrence of improved Fox operation is better than that of improved Wheeler operation, with higher satisfaction.
