摘 要:目的:探讨异体脱细胞真皮基质(acellular dermal matrix,ADM)对烧伤后期瘢痕整形的修复效果。方法:选择2016年3月-2017年3月于笔者医院进行治疗的95例烧伤后期瘢痕整形患者进行研究,随机分为观察组(n=49)和对照组(n=46)。观察组采用异体脱细胞真皮基质进行治疗,对照组采用自体中厚皮片进行治疗。比较两组二次手术率、皮片成活率、瘢痕评分、创面愈合时间、愈合率及术后瘢痕增生程度。结果:治疗后,观察组患者二次手术率、创面愈合时间显著低于对照组,皮片成活率、10d创面愈合率显著高于对照组,差异有统计学意义(P<0.05);观察组患者柔软度、血管分布、厚度及色泽评分显著优于对照组,差异有统计学意义(P<0.05);治疗后1个月,观察组重度、中度增生显著低于对照组,轻度增生显著高于对照组,差异有统计学意义(P<0.05)。结论:采用异体脱细胞真皮基质行烧伤后期瘢痕整形修复效果较好,值得推广应用。Objective To study Curative efficacy of Allogeneic acellular dermal matrix in treatment of post burn scar plastic surgery. Methods 95 cases of patients with postburn scar plastic surgery were selected for treatment in our hospital from March 2016 to March 2017, were randomly divided into the observation group (n =49) and the control group (n =46). The observation group was treated with allogenic acellular dermal matrix. The control group was treated with autologous medium thick skin flakes. Second operation rate, survival rate of skin flakes, scar score, wound healing time, healing rate and postoperative scar hyperplasia were compared between the two groups. Results After treatment, the second operation rate and wound healing time in the observation group were significantly lower than those in the control group, and the survival rate of skin graft and wound healing rate in the 10th day were significantly higher than those in the control group, the differences were statistically significant(P <0.05). The scores of softness, vascular distribution, thickness and color in the observation group were significantly better than those in the control group(P <0.05). One month after treatment, severe and moderate hyperplasia in the observation group was significantly lower than those in the control group, and mild hyperplasia was significantly higher than that in the control group(P <0.05). Conclusion Allogenic acellular dermal matrix has a good effect on scar plastic repair in the late stage of burn, which is worthy of popularization and application.
