摘 要:目的探讨2型糖尿病周围神经病变与血清同型半胱氨酸之间的关系。方法选取住院的2型糖尿病患者576例,糖尿病周围神经病变组390例,非神经病变组186例,收集两组患者的性别、年龄、血压、糖尿病病程、高血压病程、二甲双胍服用情况等,计算体质量指数(BMI),测定糖化血红蛋白(HbA1c)、空腹血糖(FPG)、血清同型半胱氨酸(Hcy)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、血肌酐(Scr)、叶酸及维生素B12等。结果糖尿病神经病变组糖尿病病程、Hb A1c、FPG、LDL-C、Hcy高于非神经病变组(P<0. 05),通过校正糖尿病病程、HbA1c、LDL-C、Scr、UACR、叶酸及二甲双胍等因素,糖尿病病程、HbA1c、血清Hcy与神经病变呈正相关(P<0. 05)。在校正维生素B12后,糖尿病病程、HbA1c、血清Hcy与神经病变仍呈正相关(P<0. 05)。结论血清同型半胱氨酸浓度与2型糖尿病周围神经病变呈正相关,对早期筛查及随访2型糖尿病周围神经病变有临床价值。Objective To explore the correlation between type 2 diabetes with diabetic peripheral neurophathy and serum homocysteine. Methods 576 hospitalized cases with type 2 diabetes were selected and divided into 2 groups: diabetic peripheral neurophathy group( n = 390) and non-diabetic peripheral neurophathy group( n = 186) and the data of the cases in the 2 groups were collected,including gender,age,blood pressure,the course of diabetes,the course of hypertension and the application of metformin hydrochloride and the BMI of all the cases were calculated;detection was made to the levels of HbAlc,FPG,Hcy,LDL-C,HDL-C,Scr,folic acid and vitamin B12 of the cases.Results The course of diabetes and the levels of Hb Alc,FPG,Hcy and LDL-C of the cases in diabetic peripheral neurophathy group were longer and higher than those of the cases in non-diabetic peripheral neurophathy group( P<0. 05),after statistical adjustment of the factors,the course of diabetes,the levels of Hb Alc,LDL-C,Scr,folic acid and vitamin B12 were in positive correlation with neurophathy( P < 0. 05);after statistical adjustment of the vitamin B12,the course of diabetes,the levels of HbAlc and Hcy were still in positive correlation with neurophathy( P<0. 05). Conclusions The concentration of serum homocysteine is in positive correlation with type 2 diabetes with diabetic peripheral neurophathy,which is of certain clinical value in the early screening and follow-up.
