摘 要:目的探讨MTHFR基因型对心血管疾病发生肾损伤的预测价值。方法入选已确诊心血管疾病的患者128例,收集其样本进行相关实验室指标检测和MTHFR基因芯片检测,对MTHFR基因型、并发症和相关检测指标之间的相关性进行分析。结果在心血管疾病患者中,基因型频率在肾损伤和对照组之间具有显著性差异。在MTHFR野生型(CC)中BUN、CREA、CYSC、CLCR、UA等肾功能相关指标明显优于突变型(CT/TT)(P <0. 05)。冠心病和MTHFR突变型同为心血管病患者中发生肾损伤的独立风险因子,前者风险系数为3. 078,后者的风险系数为1. 614,和肾病易感性之间有显著相关性。结论表明MTHFR基因在心血管病患者中对肾损伤发生的预测具有重要价值。研究提示MTHFR基因型的鉴定可能有利于预测和规避由心血管病患者的异常代谢功能引起的肾损伤。Objective To explore the value of methylenetetrahydrofolate reductase( MTHER) genotypes in predicting the risk of nephropathy in cases with cardiovascular diseases( CVD). Methods 128 cases with confirmed diagnosis of CVD were enrolled and the samples were collected;related lab tests and MTHER gene chip detection were performed;the correlation of MTHER genotype and complications to related detection index was analyzed. Results There existed an obvious difference in genotype frequency between nephropathy and the control;the related renal function index such as BUN,CREA,CYSC,CLCR and UA of wild type MTHER were much superior to those of mutant MTHER( P<0. 05);coronary heart disease and mutant MTHER were the independent risk factors of nephropathy in cases with CVD,the risk coefficient was 3. 078 and 1. 614 respectively,and mutant MTHER was closely correlated with nephropathy susceptibility. Conclusions MTHER genotype is of important value in predicting nephropathy in cases with CVD,and the identification of MTHER genotype may be of help in predicting and avoiding nephropathy induced by abnormal metabolism.
