侧卧位单侧椎弓根旁入路经皮椎体成形术治疗骨质疏松性椎体压缩骨折合并肺通气功能障碍的临床疗效

摘　　要：目的观察侧卧位单侧椎弓根旁入路经皮椎体成形术(PVP)治疗骨质疏松性椎体压缩骨折(OVCF)合并肺通气功能障碍患者的临床疗效。方法回顾性分析采用侧卧位PVP治疗的82例OVCF合并肺通气功能障碍患者的临床资料。观察组(n=42)采用侧卧位经椎弓根旁入路,对照组(n=40)采用侧卧位经椎弓根入路。比较两组患者的手术时间、X线透视次数、骨水泥渗漏率及术前、术后3 d、术后6个月的疼痛视觉模拟量表(VAS)评分和Oswestry功能障碍指数(ODI)评分。结果观察组手术时间、X线透视次数均少于对照组(均P<0.05),但两组骨水泥用量及骨水泥渗漏率比较,差异均无统计学意义(均P>0.05)。两组的VAS评分及ODI评分比较,差异均无统计学意义(均P>0.05)。结论侧卧位单侧经椎弓根旁入路与经椎弓根入路PVP术治疗OVCF并肺通气功能障碍患者手术效果相当,均能有效缓解患者的临床症状,但经椎弓根旁入路手术时间更短,术中X线照射次数更少。Objective To observe the clinical efficacy of lateral percutaneous vertebroplasty(PVP) via unilateral extrapedicular approach on osteoporotic vertebral compression fracture(OVCF) patients complicated with pulmonary ventilation dysfunction. Methods The clinical data of 82 patients,who suffered from OVCF complicated with pulmonary ventilation dysfunction and underwent lateral PVP,were analyzed retrospectively.The observation group( n =42) used lateral position and extrapedicular approach,while the control group( n =40) used lateral position and pedicular approach.Operation duration,number of X-ray fluoroscopy,and bone cement leakage rate were compared between the two groups,along with pain Visual Analogue Scale(VAS) scores and Oswestry dysfunction index(ODI) scores preoperatively,months 3 and 6 postoperatively. Results The operation duration and number of X-ray fluoroscopy in the observation group were less than those in the control group(all P <0.05),but there was no statistically significant difference in bone cement consumption or leakage rate between the two groups(all P >0.05).No statistically significant difference was found in VAS score or ODI score between the two groups(all P >0.05). Conclusion Unilateral extrapedicular approach applied to lateral PVP obtains similar surgical efficacy as compared to unilateral pedicular approach for OVCF patients complicated with pulmonary ventilation dysfunction,both can effectively alleviate patients′ clinical symptoms,however,the former achieves a shorter operation duration and minor number of intraoperative X-ray irradiation.
