神经内镜在三叉神经微血管减压术中的应用价值

摘　　要：目的探讨在神经内镜辅助下经乙状窦后入路进行微血管减压术治疗三叉神经痛的效果。方法对诊断及手术指征明确的35例原发性三叉神经痛患者进行神经内镜辅助下三叉神经微血管减压术,分析治疗结果。结果 35例患者中,三叉神经痛症状完全缓解者32例(91.4%),明显减轻者3例(8.6%);术后轻度面瘫者2例(5.7%),短暂头晕伴恶心、呕吐者3例(8.6%),发热者2例(5.7%);开颅过程中无窦损伤,术后无继发颅内出血及小脑挫伤病例。结论神经内镜辅助下经乙状窦后入路进行微血管减压术治疗三叉神经痛具有安全性高、视野好、无死角的特点,能取得满意的效果。Objective To investigate the effect of microvascular decompression via retrosigmoid approach in the treatment of trigeminal neuralgia,which was assisted by neuroendoscopy.Methods Thirty-five patients suffered trigeminal neuralgia with clear diagnosis and surgical indications underwent microvascular decompression assisted by neuroendoscope,and the treatment result was analyzed.Results Among the 35 patients,32 cases (91.4%) with symptoms of trigeminal neuralgia were completely relieved,3 cases (8.6%) were significantly alleviated;2 cases (5.7%) suffered facial paralysis,3 cases (8.6%) had transient dizziness accompanied by nausea and vomiting and 2 cases (5.7%) with fever after operation.There was no sinus injury in operation,no subsequent intracranial hemorrhage and no cerebellar contusion after operation.Conclusion The microvascular decompression via retrosigmoid approach assisted by neuroendoscope in the treatment of trigeminal neuralgia has characterstics like high safety,good view and no blind angle,which can obtain satisfied effect.
