寻常型银屑病患者血清microRNA-135a水平与Th1/Th2平衡的关系

摘　　要：【目的】分析寻常型银屑病患者血清微小RNA-135a(microRNA-135a,miR-135a)表达水平与Th1/Th2 平衡的关系。【方法】将2017 年5 月至2018 年6 月在本院诊治的63 例寻常型银屑病患者作为病例组,同期60 例健康者作为对照组,采用实时荧光定量PCR(real-time quantitative PCR,RT-qPCR)法检测两组受试者血清miR-135a 相对表达量,采用银屑病皮损面积和严重程度评分(psoriasis area and severity index,PASI)评估病例组患者病情严重程度,采用流式细胞术检测外周血CD4+ T 细胞中Th1、Th2 细胞比例,ELISA 法检测血清干扰素-γ(interferon-γ,IFN-γ)、白细胞介素2(interleukin-2,IL-2)及白细胞介素4(interleukin-4,IL-4)、白细胞介素10(interleukin-10,IL-10)水平,分析病例组血清miR-135a 表达水平与PASI 评分、Th1/Th2 的关系。【结果】病例组血清miR-135a 相对表达量低于对照组,与PASI 评分呈负相关关系(r=-0.714,P=0.005);病例组Th1 细胞比例及血清INF-γ、IL-2 水平高于对照组,Th2 细胞比例及血清IL-4、IL-10 水平低于对照组,Th1/Th2 水平高于对照组(P<0.05);病例组血清miR-135a 表达水平与Th1/Th2 水平呈负相关关系(r=-0.675,P=0.013)。【结论】寻常型银屑病患者血清miR-135a 表达降低,与Th1/Th2 失衡有关,可能通过影响Th1/Th2 平衡,参与病情发生及发展过程。【Objective】To analyze the relationship between the expression of serum microRNA-135a (miR-135a) and Th1/Th2 balance in patients with psoriasis vulgaris.【Methods】From May 2017 to June 2018,63 patients with psoriasis vulgaris treated in our hospital were selected in the case group,and 60 healthy persons were selected in the control group.The real-time quantitative PCR (RT-qPCR) method was used to detect the relative expression of serum miR-135a in both groups.The psoriasis area and severity index score (PASI score) was used to assess the severity of the disease in patients with psoriasis.The proportions of Th1 and Th2 cells in CD4+ T cells of peripheral blood were detected by flow cytometry.The levels of serum interferon-γ(IFN-γ),interleukin-2 (IL-2),interleukin-4 (IL-4) and interleukin-10 (IL-10) were detected by ELISA,and the relationships of serum miR-135a level with PASI score and Th1/Th2 were analyzed respectively.【Results】The relative expression of serum miR-135a in the case group was lower than that in the control group, which was negatively correlated with PASI score (r=-0.714,P=0.005).The ratio of Th1 cells and the levels of serum INF-γ and IL-2 in the case group were higher than those in the control group,while the ratio of Th2 cells and the levels of serum IL-4 and IL-10 were lower than those in the control group,and the level of Th1/Th2 was higher than that in the control group (P<0.05).There was a negative correlation between the levels of serum miR-135a and Th1 / Th2 (r=- 0.675,P=0.013).【Conclusion】The serum expression of miR- 135a in patients with psoriasis vulgaris decreases,which is related to Th1 / Th2 imbalance.It may participate in the occurrence and development of the disease by affecting the balance of Th1/Th2.
