银屑病患者尿酸排泄分数与冠状动脉硬化的相关性研究

摘　　要：目的分析银屑病患者尿酸排泄分数与冠状动脉硬化严重程度的相关性。方法选取2015年12月—2017年12月于南阳市中心医院门诊就诊及住院的银屑病患者282例。根据Gensini积分结果将研究对象分为0或1组分84例、2～20分组79例及>20分组119例,比较各组尿酸排泄分数水平。采用Spearman相关性分析Gensini积分与各参数间的关系,采用多元线性回归分析Gensini积分的影响因素。结果 >20分组总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)及尿酸(SUA)高于其余两组(P<0.05),高密度脂蛋白胆固醇(HDL-C)和FEUA低于其余两组(P<0.05);Gensini积分2～20分组TC、LDL-C、SUA高于0～1分组(P<0.05),HDL-C和FEUA低于0～1分组(P<0.05)。Spearman相关分析显示,Gensini积分与TC、LDL-C及SUA呈正相关(r_s=0.495、0.660和0.533,P=0.011、0.000和0.006),与HDL-C、FEUA呈负相关(r_s=-0.708和-0.775,均P=0.000)。Gensini积分与TC、LDL-C及SUA呈正相关(r_s=0.495、0.660和0.533,P<0.05),与HDL-C、尿酸排泄分数呈负相关(r_s=-0.708和-0.775,P<0.05)。LDL-C、HDL-C及尿酸排泄分数为Gensini积分的影响因素(P<0.05)。结论银屑病患者尿酸排泄分数与冠状动脉硬化严重程度呈正相关,尿酸排泄分数可能是冠状动脉硬化的危险因素之一。Objective To analyze the correlation between fraction excretion of uric acid (FEUA) and severity of coronary artery lesion in patients with psoriasis. Methods According to the Gensini score, the 282 patients with psoriasis were divided into 0 - 1 score group (n = 84), 2 - 20 score group (n = 79) and >20 score group (n = 119), and FEUA level were compared among three groups. Spearman correlation analysis was used to analyze the relationship between Gensini score and other index, and multiple linear regression was used to analyze the influencing factors of Gensini score. Results ① TC, LDL-C and SUA were higher, but HDL-C and FEUA were lower in Gensini score > 20 group than those in Gensini score 0 - 1 group and Gensini score 2 - 20 group (P < 0.05);TC, LDL-C and SUA were higher, but HDL-C and FEUA were lower in Gensini score 2 - 20 group than those in Gensini score 0 - 1 group (P < 0.05).② Spearman correlation analysis showed that the Gensini score was positively related with TC, LDL-C and SUA (r s =0.495, 0.660, 0.533, P =0.011, 0.000, 0.006), and negatively related with HDL-C and FEUA (r s =-0.708,-0.775, all P =0.000). Multiple linear regression analysis showed LDL-C, HDL-C and FEUA were associated with Gensinis score (P < 0.05). Conclusions FEUA level is associated with severity of coronary artery lesion in patients with psoriasis, and maybe FEUA is one of risk factors for coronary artery lesion.
