凉血化瘀方加减治疗原发性血小板增多症单病例随机对照研究

摘　　要：目的观察凉血化瘀方加减治疗原发性血小板增多症的疗效。方法运用单病例随机对照试验方法,先通过预初试验,确定1例原发性血小板增多症患者的观察期2周与洗脱期1周,再进行三轮次随机双盲自身对照试验。试验期给予患者凉血化瘀方加减治疗,每日1剂,同时给予西药安慰剂(维生素C,每片0.5 g)30 mg/(kg·d)口服,每日1次;对照期予羟基脲片30 mg/(kg·d)口服和中药对照方,每日1剂。比较患者三轮次试验后试验期与对照期血小板计数和骨髓增殖性肿瘤总症状评估量表(MPN-10)积分。结果三轮次试验后,试验期血小板计数下降(120.67±22.03)×10~9/L,与对照期(124.33±24.09)×10~9/L比较差异无统计学意义(P>0.05);试验期MPN-10积分下降(13.00±3.61)分,明显高于对照期(10.33±3.06)分(P<0.05)。结论凉血化瘀方加减在降低原发性血小板增多症患者血小板计数与羟基脲片相当,但在改善临床症状方面优于羟基脲片。Objective To observe the efficacy of modified Liangxue Huayu Recipe( 凉血化瘀方) in the treatment of essential thrombocytosis. Methods A n of one trial was used. Pre-experimental testing was performed to a patient with essential thrombocytopenia to determine the patient’s observation period of 2 weeks and the elution period of 1 week. Three rounds of randomized double-blind self-controlled trials were performed. During the trial period,patients were given modified Liangxue Huayu Recipe,one dose per day,and a western medicine placebo( vitamin C,0. 5 g per tablet) 30 mg/( kg·d) for oral administration,once a day. During the control period,the patients were given hydroxyurea tablets 30 mg/( kg·d) for oral administration and Chinese medicine control recipe,one dose per day. The platelet count and scores of the myeloproliferative tumor total symptom assessment scale( MPN-10) in the trial period and the control period after the 3 rounds of the trial were compared. Results After three rounds of trials,there was no significant difference in the platelet count between the trial period( 120. 67 ± 22. 03) × 10~9/L and the control period( 124. 33 ± 24. 09) × 10~9/L( P > 0. 05). The MPN-10 score during the trial period decreased( 13. 00 ± 3. 61) which was significantly higher than that in the control period( 10. 33 ± 3. 06)( P < 0. 05).Conclusion Modified Liangxue Huayu Recipe is equal to hydroxyurea tablets in reducing the platelet count in patients with essential thrombocytopenia,but it is superior to hydroxyurea tablets in improving clinical symptoms.
