摘  要:目的探讨C-反应蛋白(C reactive protein,CRP)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)及一氧化氮(nitric oxide,NO)在病毒性脑炎患儿脑脊液中的表达水平及临床意义。方法选择2012年1月至2018年3月收治的100例病毒性脑炎患儿为观察组,行健康体检的100例儿童健康志愿者为对照组。采集其脑脊液标本,检测CRP、TNF-α、NO表达水平,比较观察组与对照组的CRP、TNF-α、NO表达水平,并采用皮尔逊相关系数分析法,分析CRP、TNF-α、NO表达水平与小儿病毒性脑炎发生的相关性。根据病情严重程度将病毒性脑炎患儿分为轻症组与重症组,比较轻症组与重症组患儿CRP、TNF-α、NO表达水平,并分析CRP、TNF-α、NO表达水平与病毒性脑炎患儿病情严重程度的相关性。结果观察组CRP、TNF-α、NO表达水平均高于对照组(P<0.05);CRP、TNF-α、NO表达水平与病毒性脑炎的发生呈正相关(r值分别为0.735、0.729、0.758,P<0.01)。重症组CRP、TNF-α、NO表达水平高于轻症组(P<0.05)。CRP、TNF-α、NO表达水平与病毒性脑炎患儿的病情严重程度正相关(r值分别为0.715、0.713、0.752,P<0.01)。结论 CRP、TNF-α、NO表达水平在病毒性脑炎患儿脑脊液中普遍上调,通过检测患儿脑脊液中的CRP、TNF-α、NO可对病毒性脑炎的发生和发展予以反映,可将CRP、TNF-α、NO作为小儿病毒性脑炎早期诊断、病情评估的辅助指标。Objective To investigate the expression levels and clinical significances of C reactive protein(CRP), tumor necrosis factor-α(TNF-α) and nitric oxide(NO) in cerebrospinal fluid(CSF) in children with viral encephalitis.Methods A total of 100 children with viral encephalitis who were admitted and treated in our hospital from January 2012 to March 2018 were enrolled as observation group, and 100 healthy volunteers who underwent physical examination in our hospital were enrolled as control group. Cerebrospinal fluid(CSF) specimens were collected to detect and compare the expression levels of CRP, TNF-α, NO, CRP between the two groups. The correlation between the expression levels of CRP, TNF-α, NO and the occurrence of children viral encephalitis was analyzed by Pearson’s correlation coefficient. The children with viral encephalitis were redivided into mild case group and severe case group according to the severity of viral encephalitis. The expression levels of CRP, TNF-α, and NO were observed and compared between the mild case group and severity case group, moreover the correlation between the expression levels of CRP, TNF-α, NO and the severity of viral encephalitis was analyzed.Results The expression levels of CRP, TNF-α, and NO in observation group were significantly higher than those in control group(P<0.05). The expression levels of CRP, TNF-α, and NO were closely and positively related to the occurrence of viral encephalitis. The expression levels of CRP, TNF-α and NO in severe case group were significantly higher than those in mild case group(P<0.05). There was a positive and close correlation between the expression levels of CRP, TNF-α,NO and the severity of viral encephalitis. Conclusion The expression levels of CRP, TNF-α, and NO are generally increased in cerebrospinal fluid of children with viral encephalitis.
