单孔腹腔镜与多孔腹腔镜乙状结肠代阴道成型术患者的对照研究及护理

摘　　要：目的比较单孔腹腔镜与多孔腹腔镜乙状结肠代阴道成型术临床效果差异。方法回顾性分析妇科2010年8月至2018年4月施行全麻腹腔镜下乙状结肠代阴道成型术患者53例,其中单孔腹腔镜手术(单孔组)26例,多孔腹腔镜手术(多孔组)27例。比较2组患者手术时间、术中出血量、术后24 h疼痛评分(Prince-Henry法)、术后镇痛药物使用情况、术后体象量表(BIS)、美容评分(CS)及住院时间。结果 53例手术均全部成功。单孔组患者手术时间长于多孔组,差异有统计学意义(P<0.05);术后美容评分(CS)单孔组高于多孔组,差异有统计学意义(P<0.05)。2组患者术中出血量、镇痛药物使用例数、住院时间及术后BIS评分、术后第1天疼痛评分比较,差异均无统计学意义(P>0.05)。结论单孔腹腔镜乙状结肠代阴道手术虽较传统的多孔腹腔镜手术在出血量、减轻疼痛方面差异不大,但具有创伤小、切口美观、满意度高等优点,可以最大程度保护患者隐私,是安全、可行的手术方式。Objective To compare the e clinical effects of haplopore laparoscopic vaginoplasty and multihole laparoscopic vaginoplasty.Methods The clinical data about 53 patients who underwent laparoscopic sigmoid colon vaginoplasty under general anesthesia in our hospital from August 2010 to April 2018 were retrospectively analyzed.Among them,26 patients underwent haplopore laparoscopic vaginoplasty(haplopore group), and 27 patients underwent multihole laparoscopic vaginoplasty(multihole group). The operation duration,intraoperative blood loss,postoperative 24 h pain score(Prince-Henry method),postoperative analgesic drug use,postoperative body image scale(BIS),and cosmetic score CS) and total hospital stay were observed and compared between the two grousp.Results The surgical effects in the 53 patients were successful.The operation time in haplopore group was significantly longer than that in multihole group(P<0.05),and the postoperative cosmetic scores in haplopore group were significantly higher than those in multihole group(P<0.05).However there were no significant differences in intraoperative blood loss, analgesic use,total hospital stay,postoperative BIS scores,and postoperative pain scores on 1 day after operation between the two groups(P<0.05).Conclusion Although there are no significant differences in intraoperative blood loss and pain relief between haplopore laparoscopic sigmoid colon vaginoplasty and multihole laparoscopic sigmoid colon vaginoplasty,the former has the advantages of less trauma,better cuts,and higher satisfaction degree,which can protect the privacy of patients to the greatest extent,therefor, haplopore laparoscopic sigmoid colon vaginoplasty is a safe and possible surgical mode.
