早期同步规范化家庭康复在痉挛型脑瘫患儿运动治疗中的应用

摘　　要：目的探讨早期同步规范化家庭康复对痉挛型脑瘫患儿运动功能的影响。方法分两个阶段随机选取早期进行医院康复的痉挛型脑瘫患儿各30例为对照组及观察组,对照组采取医院常规综合康复,包括运动疗法、普通针刺及穴位贴敷、康复指导及宣教、引导式教育、营养神经药物等常规措施,其中运动治疗时家长在外等候;观察组在医院常规综合康复基础上,采取同步规范化家庭康复,措施包括课上家长陪同运动治疗、健康教育路径、心理干预、课后家庭作业、治疗师检查-督促-指导等,分别于4周、12周末比较2组运动功能状况(内收角及踝背屈角的变化)及粗大运动改善情况等康复效果。结果 2组治疗4、12周时,观察组内收角及踝背屈角改善程度明显优于对照组(P<0.05);2组治疗12周末,GMFM评分均较治疗前有明显提高(P<0.01),患儿母亲焦虑情绪改善、家庭康复知识知晓率均有明显提高,且观察组明显优于对照组(P<0.05)。结论运动治疗早期同步规范化家庭康复能明显促进脑瘫患儿运动功能的改善,有利于脑瘫患儿的预后。Objective To investigate the effects of early synchronization family standardized rehabilitation on motor function of children with spastic cerebral palsy. Methods A total of 60 children with spastic cerebral palsy were randomly divided into control group and observation group,with 30 cases in each group.The patients in both groups were treated by routine comprehensive rehabilitation therapy including exercise therapy,common acupuncture and acupuncture point applyment,rehabilitation guidance and heath education as well as trophic nerve drugs,moreover,the patients in observation group,on the basis of routine comprehensive rehabilitation therapy,were treated by early synchronization family standardized rehabilitation including parental accompanying exercise therapy,health education,psychological intervention, therapists' examination-supervision-guidance and assignments after class.At 4w, 12w after the treatment intervention,the motor function indexes including adduction angle and ankle dorsal flexion angle as well as gross exercise improvement were observed and compared between the two groups. Results At 4w, 12w after the treatment intervention,the improvement degree of adduction angle and ankle dorsal flexion angle in observation group was superior to that in control group ( P <0.05),and the GMFM scores in both groups were significantly increased,as compared with those before treatment ( P <0.01).The improvement degree of anxiety emotion of patient's mother and the awareness rate of family rehabilitation knowledge in observation group were superior to those in control group ( P <0.05). Conclusion The early synchronization family standardized rehabilitation can significantly improve the motor function of children with spastic cerebral palsy,which is conducive to the prognosis of children with cerebral palsy.
