摘 要:目的观察代谢综合征(MS)患儿血清上皮型脂肪酸结合蛋白(E-FABP)表达水平及其临床意义。方法前瞻性纳入2016年1月至2017年2月于邢台市人民医院收治的49例MS患儿为观察组,另取同期在本院接受疫苗注射的健康儿童50例作为正常对照组。入院后即刻采集两组患儿空腹外周静脉血标本,采用双抗体夹心酶联免疫吸附法检测血清E-FABP表达水平。比较两组患儿E-FABP表达水平,分析两组糖代谢指标血空腹血糖(FBG)、胰岛素抵抗指数(HOMA-IR)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、非高密度脂蛋白胆固醇(non-HDL-C)、载脂蛋白A1(ApoA1)、载脂蛋白B(apo B)水平,观察内皮损伤指标一氧化氮(NO)、内皮素-1(ET-1)水平及炎症反应指标白介素-1β(IL-1β)、白介素6(IL-6)、白介素17(IL-17)、C反应蛋白(CRP)水平,并采用Pearson检验分析E-FABP表达水平与上述指标的关系。结果观察组E-FABP、HOMA-IR、FBG、TC、Ap B、LDL-C、non-HDL-C显著高于正常对照组(P <0. 05),HDL-C、ApoA1显著低于正常对照组(P <0. 05)。观察组ET-1、IL-1β、IL-6、IL-17、CRP显著高于正常对照组(P <0. 05),NO显著低于正常对照组(P <0. 05)。经直线相关分析显示,MS患儿E-FABP表达水平与炎症反应指标IL-1β、IL-6、IL-17、CRP水平均呈线性正相关(P <0. 05);与内皮损伤指标NO水平呈线性负相关(P <0. 05),与ET-1水平呈线性正相关(P <0. 05);与糖脂代谢指标HOMAIR、FBG、TC、Ap B、LDL-C、non-HDL-C均呈线性正相关(P <0. 05),与ApoA1、HDL-C水平均呈线性负相关(P <0. 05)。结论 MS患儿血清E-FABP水平较正常儿童明显升高,且E-FABP的表达水平与炎症反应、内皮损伤及糖脂代谢指标密切相关,临床上应引起足够重视。Objective In order to observe the expression of serum epithelial fatty acid binding protein(E-FABP)in children with metabolic syndrome(MS)and its clinical significance.Methods 49 children with MS admitted to our hospital from January 2016 to February 2017 were selected as observation group.Another 50 healthy children who received vaccination in our hospital during the same period were selected as normal control group.Immediately after admission,fasting peripheral venous blood samples were collected.Serum expression of E-FABP was detected by double antibody sandwich ELISA.Blood sugar metabolism indices including fasting blood glucose(FBG),insulin resistance index(HOMA-IR),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),non-high-density lipoprotein cholesterol(non-HDL-C),apolipoprotein A1(apoA1),apolipoprotein B(apoB)were analyzed.Endothelial injury indices including nitric oxide(NO),endothelin-1(ET-1)and IL-1β,IL-6,IL-17 and c-reactive protein(CRP)were observed.The relationship between expression level of E-FABP and above indices were analyzed by Pearson test.Results E-FABP,HOMA-IR,FBG,TC,apoB,LDL-C and non-HDL-C in observation group were significantly higher than control group(P<0.05),while HDL-C and apoA1 were significantly lower than control group(P<0.05).ET-1,IL-1β,IL-6,IL-17 and CRP in observation group were significantly higher than control group(P<0.05),while NO was significantly lower than control group(P<0.05).Linear correlation analysis showed that E-FABP expression in children with MS presented positive linear correlation with inflammatory response indicators such as IL-1β,IL-6,IL-17 and CRP levels(P<0.05),a linear negative correlation with endothelial injury index NO level(P<0.05),a positive linear correlation with ET-1 levels(P<0.05),positive linear correlations with metabolic indices such as HOMA-IR,FBG,TC,apoB,LDL-C,non-HDL-C(P<0.05),and negative linear correlations with apoA1 and HDL-C levels(P<0.05).Conclusion The expression of E-FABP
