升-降主动脉旁路术在成人复杂主动脉缩窄中的应用

摘　　要：目的探讨升-降主动脉人工血管旁路术治疗成人复杂主动脉缩窄的手术效果并总结其临床经验.方法 2015年10月至2018年7月同济大学附属东方医院心外科应用升主动脉-降主动脉人工血管转流术治疗成人复杂主动脉缩窄2例,均为男性,年龄分别为22岁和46岁.两例患者均经桡动脉、足背动脉穿刺测压,根据桡动脉和足背动脉手术前后平均压差变化评价手术效果.结果术后均治愈出院.术前桡动脉和足背动脉平均压差分别为48 mmHg和55 mm Hg;术后桡动脉和足背动脉平均压差6mmHg和9mmHg,较术前明显缩小.术后主动脉CTA复查示转流人工血管通畅.结论升-降主动脉人工血管旁路术是治疗成人复杂主动脉缩窄的有效手段.Objective To investigate the surgical effect of adult complex aortic coarctation using ascending -descending aortic artificially grafting bypass and summarize the clinical experience. Methods From October 2015 to July 2018, ascending-descending aortic artificially grafting bypass was performed on 2 adult patients with complex aortic coarctation, of whom both male, aged 22 and 46, respectively. Both patients were punctured through radial artery and dorsal pedal artery to measure the pressure. The operation effect was evaluated according to the mean pressure difference of radial artery and dorsal pedal artery before and after operation. Results Two patients were cured after operation. The mean pressure difference of radial artery and dorsal radial artery was 48 mm Hg and 55 mm Hg before operation and 6 mm Hg and 9 mm Hg at 24-hour after operation, which was significantly smaller than that before operation. Postoperative CTA examination of the aorta showed that the bypass graft was unobstructed. Conclusion Ascending-descending aorta artificial vascular bypass would be an effective means for the treatment of adult complex aortic coarctation patients.
