摘 要:目的探讨声触诊组织量化成像(VTIQ)技术在鉴别颈部淋巴结疾病中的应用价值。方法回顾性分析经超声引导下穿刺活检确诊的88例患者,共105个颈部淋巴结的VTIQ图像资料。在VTIQ速度模式下密集测量淋巴结内剪切波速度(SWV),获取最大值(Vmax)、平均值(Vmean)、最小值(Vmin)及四分位距(V_(IQR))。构建ROC曲线确定准确性最高的SWV并获得最佳鉴别诊断截断值。结果 105个颈部淋巴结中包含反应性淋巴结28个,转移性淋巴结54个,淋巴瘤23个。三组淋巴结上述指标差异均具有统计学意义(P<0.001);依据ROC曲线分析结果,SWV测值中以Vmax的诊断效能最佳。结论 VTIQ技术在鉴别颈部淋巴结疾病中具有一定的应用价值,并以Vmax的诊断效能最佳。Objective To investigate the value of Virtual Touch tissue quantification(VTIQ)technique in the differential diagnosis of cervical lymphadenopathy.Methods The VTIQ imaging data of 105 cervical lymph node in 88 patients diagnosed by ultrasound-guided core biopsy were retrospectively analyzed.The shear wave velocity(SWV)of the lymph nodes was measured intensively in the velocity mode and maximum(Vmax),mean(Vmean),minimum(Vmin)and interquartile range(VIQR)of SWV was obtained.The receiver operating characteristic(ROC)curve was generated to determine the most accurate SWV value and the cut-off value for differential diagnosis of cervical lymphadenopathy.Results 105 cervical lymph nodes included 28 reactive lymph nodes,54 metastatic lymph nodes,and 23 lymphomas.The differences of Vmax,Vmean,Vmin and VIQR were significantly among the three groups(P<0.001).According to ROC curve analysis,the Vmax was best value for the diagnosis efficacy.Conclusions VTIQ technique has important application value in the differential diagnosis of cervical lymphadenopathy,and the Vmax is the best parameter for differential diagnosis.
