摘  要:目的探讨超声造影(CEUS)联合实时弹性成像(RTE)鉴别诊断浅表肿大淋巴结良恶性的价值。方法回顾性分析我科行超声检查57例患者共83枚浅表淋巴结,入选淋巴结均行CUS、RTE和CEUS检查,并取得病理结果。结果淋巴结造影增强模式:良性以均匀增强为主,恶性以非均匀增强为主;造影剂填充模式:良性以离心性增强为主,恶性以向心性增强为主。CUS、RTE、CEUS以及CEUS联合RTE诊断的灵敏度、特异度及符合率分别为71.7%、78.4%、74.7%;87.0%、83.8%、85.5%;93.5%、86.5%、90.4%;97.8%、81.1%、90.4%。结论 RTE和CEUS较CUS在鉴别诊断浅表淋巴结良恶性具有优势,两种新技术联合可提高诊断灵敏度。Objective To explore the clinical value of combined ultrasonography and real-time elastography in the differential diagnosis of superficial lymph nodes.Methods A total of 83 superficial enlarged lymph nodes were retrospectively analyzed in 57 patients undergoing ultrasound examination in our department.All lymph nodes were examined by CUS,RTE and CEUS,and surgical or puncture pathological results were obtained.Results Contrast enhancement pattern of lymph nodes:Benign is given priority to with homogeneous enhancement,malignant is given priority to with heterogeneous enhancement;Contrast filling mode:Benign was mainly the enhancement of alienation,The main malignancy is the enhancement of centrality.Compared with the pathological results,the sensitivity,specificity and coincidence rate of CUS,RTE,CEUS and CEUS combined with RTE were 71.7%,78.4%,74.7%;87.0%,83.8%,85.5%;93.5%,86.5%,90.4%;97.8% 81.1%,90.4%.Conclusions Compared with CUS,RTE and CEUS have advantages in differentiating benign and malignant superficial lymph nodes,and combination of the two new techniques can improve the diagnostic sensitivity.
