摘　　要：目的探讨潜突型舌下腺囊肿的声像图特征。方法回顾性分析我院经手术病理证实的17例潜突型舌下腺囊肿的超声表现。结果 17例潜突型舌下腺囊肿患者超声准确诊断14例,诊断符合率82.4%。16例病灶位于颌下区,1例位于颏下区;囊肿最大径21～71mm;13例外形不规则,由内前方向后外侧逐渐变大,呈"漏斗状"改变,4例呈椭圆形;13例未见明显包膜,4例可见包膜样结构;12例呈无回声,5例呈低回声或囊实混合回声;彩色多普勒检查16例内部未见明显血流信号,1例内部伴结节可见血流信号;17例患者均可见下颌舌骨肌回声连续性中断。结论潜突型舌下腺囊肿的超声表现有一定的特异度,典型表现为颌下区"漏斗状"无回声,无包膜,内部无血流信号,周围下颌舌骨肌回声连续性中断。Objective To investigate the ultrasonic manifestations of plunging ranulas.Methods Ultrasonogram features of 17 cases of plunging ranulas proved with operation and pathology were retrospectively analyzed.ResultsFourteen cases of plunging ranulas were correctly diagnosed by ultrasound.The accurate rate was 82.4%.Among 17 patients,lesions located in the submandibular region in 16 patients,while in the submental area in 1 patient.The lesions maximum diameters ranged from 21 mm to 71 mm.Irregular shape lesions were found in 13 cases,and it gets bigger and bigger from the anteromedial to posterolateral,funnel-shaped changed.Ellipse shaped lesions were found in 4 cases.There were 13 cases without obvious capsule and 4 cases with envelope structure.Twelve cases showed anechoic and 5 cases were hypoechoic or blended echoic.Color doppler flow imaging showed no obvious blood flow signal in16 cases.Blood flow signal was observed in 1 patient of plunging ranula with nodule.All the 17 patients showed the mylohyoid defects.Conclusions Ultrasonographic of plunging ranulas have characters.The typical performance is "funnelform" with anechoic in the submandibular space,nonencapsulated.There is no blood flow signal in the lesions and adjacency mylohyoid defects.
