关节镜下保留残端重建术治疗前交叉韧带损伤的疗效观察

摘　　要：目的观察关节镜下保留残端重建术治疗前交叉韧带(ACL)损伤的效果。方法选取临汾市人民医院2015年5月至2018年5月收治的60例ACL损伤患者为研究对象。按手术方法将其分为观察组和对照组,每组30例。观察组给予关节镜下保留残端重建术,对照组给予关节镜下切除残端重建术,比较两组膝关节功能评分(Lyshplm评分、IKDC评分)、膝关节本体感觉功能(被动角度试验、本体感觉恢复阈值)及KT-2000前移距离。结果术后,观察组Lyshplm评分、IKDC评分均高于对照组(P均<0.05);观察组KT-2000前移距离低于对照组(P<0.05);观察组被动角度试验、本体感觉恢复阈值均低于对照组(P均<0.05)。结论ACL损伤应用关节镜下保留残端重建术疗效显著,可有效改善术后膝关节稳定性,促进本体感觉恢复,值得进一步推广。Objective To observe the efficacy of arthroscopic reconstruction with remnant preservation on anterior cruciate ligament (ACL) injury. Methods Sixty patients with ACL injury in the People’s Hospital of Linfen from May 2015 to May 2018 were selected as the research subjects. According to the surgical methods, they were divided into observation group and control group, with 30 cases in each group. Patients in observation group were given arthroscopic reconstruction with remnant preservation, and patients in control group were given arthroscopic reconstruction with remnant excision. Knee function score (Lyshplm score, IKDC score), knee proprioceptive function (passive angle test, proprioceptive recovery threshold) and forward distance of KT-2000 of the two groups were compared. Results The Lyshplm score and IKDC score of observation group were significantly higher than those of control group after treatment (P<0.05). The forward distance of KT-2000 in observation group was significantly lower than that in control group after treatment (P<0.05). The passive angle test, proprioceptive recovery threshold of observation group were significantly lower than those of control group after treatment (P<0.05). Conclusions The efficacy of arthroscopic reconstruction with remnant preservation for ACL injury is significant. It can effectively improve the stability of knee joint and promote the recovery of proprioception. So it is worth further promotion.
