摘 要:目的探讨维持性血液透析(maintenance hemodialysis,MHD)患者血清25羟维生素D [25(OH) D]的表达水平对肾性贫血的程度以及红细胞生成素(erythropoietin,EPO)治疗反应的影响。方法选取本院维持规律血液透析患者103例,根据25(OH) D水平分为不足组(30例)、缺乏组(38例)和严重缺乏组(35例),检测全部患者血清25(OH) D、血红蛋白(Hb)、铁蛋白(SF)、血肌酐(Scr)、血钙(Ca)、磷(P)、白蛋白(ALB)、全段甲状旁腺激素(i PTH)、超敏C反应蛋白(hs CRP)等指标,计算红细胞生成素抵抗指数(erythropoietin resistance index,ERI),比较各组临床指标并进行相关性分析。结果各组随着25(OH) D水平的下降,Hb逐渐降低,组间差异有统计学意义(P<0.05);相关性分析:25(OH) D缺乏与Hb呈正相关(P<0.05),与ERI、i PTH、hs CRP呈负相关(P<0.05); ERI与i PTH、hs CRP呈正相关(P<0.05)。结论血清25(OH) D缺乏对MHD患者贫血以及EPO低反应的影响可能与甲状旁腺功能亢进以及微炎症有关。Objective To investigate the effect of serum 25 hydroxyl vitamin D[25( OH) D]expression on renal anemia and erythropoietin( EPO) treatment response in patients with maintenance hemodialysis( MHD).Methods A total of 103 patients received regular hemodialysis were selected from the first hospital affiliated to Jiamusi University. According to the level of 25( OH) D,the group was divided into insufficient group( 30),deficient group( 38) and severely deficient group( 35). The serum 25( OH) D,hemoglobin( Hb),ferritin( SF),serum creatinine( Scr),serum calcium( Ca),phosphorus( P),albumin( ALB),whole parathyroid hormone( i PTH),hypersensitive c-reactive protein( hs CRP) and other indicators were measured in all patients. The erythropoietin resistance index( ERI) was calculated,and the clinical indexes of each group were compared and analyzed. Results With the decrease of 25( OH) D level,Hb gradually decreased in each group,and the difference was statistically significant( P < 0. 05). Correlation analysis should that 25( OH) D deficiency was positively correlated with Hb( P<0.05),and negatively correlated with ERI,i PTH and hs CRP( P<0.05).ERI was positively correlated with i PTH and hs CRP( P< 0.05). Conclusions The effects of serum 25( OH) D deficiency on anemia and low EPO response in MHD patients may be associated with hyperparathyroidism and micro-inflammation.
