老年重症患者肠内营养相关性腹泻的原因分析与护理对策

摘　　要：目的探讨老年重症患者采用肠内营养方案治疗期间发生腹泻的原因及护理对策。方法选择2017年2月至2018年2月我院收治的202例行肠内营养治疗的老年重症患者为研究对象,统计肠内营养相关性腹泻的发生情况并进行度因素分析,总结原因,制定护理对策。结果 202例老年行肠内营养支持治疗的老年重症患者中,低蛋白血症、抗生素使用、肠内营养液剂量、供给速度、肠内营养比较差异有统计学意义(P <0. 05),两组性别比较差异无统计学意义(P> 0. 05)。行多因素logistic回归分析显示,低蛋白血症、抗生素使用、肠内营养液剂量、供给速度、肠内营养时间均为诱导肠内营养相关性腹泻的独立危险因素。结论低蛋白血症、抗生素使用、肠内营养液剂量、供给速度、肠内营养时间是老年重症患者肠内营养相关性腹泻发生的独立危险因素,应针对这些因素给予护理干预。Objective To explore the causes and nursing strategies of diarrhea during the treatment of elderly patients with severe disease by enteral nutrition.Methods A total of 202 elderly patients with severe enteral nutrition treatment who were admitted to our hospital from February 2017 to February 2018 were selected as the study subjects.The incidence of enteral nutrition-related diarrhea was analyzed.What’s more,we also analyzed the factors,and developed nursing strategies for it.Results Among the 202 elderly patients with severe disease by enteral nutrition support treatment,there was significant statistical difference in hypoproteinemia,antibiotic use,enteral nutrient solution dose,supply rate,and enteral nutrition(P<0.05).There was no significant statistical difference in the comparison of sexual between the two groups(P>0.05).Multivariate logistic regression analysis showed that hypoproteinemia,antibiotic use,enteral nutrient solution dose,supply rate and enteral nutrition time were independent risk factors for induction of enteral nutrition-related diarrhea.Conclusion Hypoproteinemia,antibiotic use,enteral nutrient solution dose,supply rate and enteral nutrition time are independent risk factors for enteral nutrition-related diarrhea in elderly patients with severe disease.We should take nursing intervention according to these factors.
